张玉琴从郁(瘀)毒损络分期辨治糖尿病肾病

摘　　要：张玉琴教授认为"久病入络"和"久痛入络",郁(瘀)毒损伤肾络为糖尿病肾病的主要病机,提出了消渴郁(瘀)毒学说,"郁"在前,"瘀"在后,"郁"范围广,"瘀"比较局限;"郁"指气郁,"瘀"指血瘀。气郁、酒郁、食郁最终导致血瘀,运用解毒通络法治疗。早期以气阴两虚、瘀血阻络为主,益气养阴,活血即是解毒;终末期则以阴阳两虚、湿毒上逆突出,形成"痰瘀""湿瘀"等,温阳降浊、化瘀利湿即是解毒。通络之法,以化瘀通络、利湿通络及虫类药搜风通络为主。Professor Zhang Yuqin believes that＂prolonged illness into the collaterals＂and＂prolonged pain into the collaterals＂, Yu（blood stasis） toxic kidney injury network main disease is diabetic nephropathy machine,Thirst waste Yu（blood stasis） proposed toxin theory,＂Qi depression＂in front,＂blood stasis＂as follows,＂Qi depression＂is wide-ranging,＂blood stasis＂is limited,＂Qi depression＂refers to Qi stagnation,＂blood stasis＂ finger blood stasis.Qi stagnation, wine, food stagnation eventually lead to blood stasis, the use of detoxicate and unblock collaterals therapy.At the early stage of deficiency of both Qi and Yin, blood stasis based,supplementing Qi and nourishing Yin,blood circulation is detoxification;In terminal stage deficiency, yin and Yang damp on the inverse prominent,the formation of phlegm, dampness and blood stasis,warming Yang and descending the turbid, removing stasis and dampness is detoxification。The collateral law, to remove blood stasis and dampness so that unblock collaterals, and insect drugs mainly search wind and dredging collaterals。
