阿维A联合雷公藤及他卡西醇软膏治疗中重度银屑病的临床疗效

摘　　要：目的探讨阿维A联合雷公藤口服及他卡西醇软膏外用治疗中重度银屑病的临床疗效以及安全性。方法将79例中重度银屑病患者进行回顾性分析,并按不同治疗方法将其分为三组:雷公藤组23例、阿维A组26例以及阿维A联合雷公藤组30例,每组均外用他卡西醇软膏。治疗前后均进行PASI评分,治疗后计算有效率,并记录不良反应,比较三组疗效以及安全性。结果治疗前三组PASI分值差异不显著(P>0.05)。三组治疗4周及8周,PASI分值与治疗前比较均明显下降(P<0.001)。治疗4周时联合组PASI分值与雷公藤组、阿维A组比较显著下降(P<0.05)。治疗4周时联合组有效率明显高于雷公藤组(P<0.05);治疗8周时联合组有效率明显高于雷公藤组(P<0.05)及阿维A组(P<0.05)。三组间不良反应发生率无显著性差异(P>0.05)。结论阿维A联合雷公藤及他卡西醇软膏治疗中重度银屑病效果优于单一治疗。而且这种联合治疗安全性较好。Objective To assess the clinical efficacy and the safety of oral acitretin capsules combined with tripterygium glycosides tablet and topical tacalcitol ointment in treatment of moderate to severe psoriasis. Methods Seventy-nine cases of moderate to severe psoriasis were retrospectively analyzed, and divided into three groups according to the different treatment： oral tripterygium glycosides tablet group（n =23）,oral acitretin capsules group（ n = 26） and oral acitretin capsules combined with tripterygium glycosides tablet group（n = 30）. All patients were given topical tacalcitol ointment. PASI scores were performed before and after treatment. Effective rate was calculated. The adverse reactions were recorded during teatment. The efficacy and safety among three groups were compared. Results There was no significant difference in PASI scores before treatment among three groups （P 〉 0.05）. PASI scores signifi- cantly decreased at 4, 8 week after treatment in each group （P 〈 0. 001 ）. PASI scores in combination group decreased significantly compared with the other two groups at 4 week after treatment（P 〈 0.05 ）. Effective rate was significantly higher in combination group than in tripterygium group at 4 week after treatment（P 〈 0.05 ）. Effective rate in combination group was significantly higher than in the other two groups at 8 week after treatment （ P 〈 0.05 ）. There was no significant difference in incidence of adverse effects among three groups（P 〉 0.05）. Conclusion The oral acitretin capsules combined with tripterygium glycosides tablet and topical tacalcitol oint- ment for moderate to severe psoriasis is safe, and its efficacy is better than that of monotherapy.
