摘 要:目的:了解中心体异常扩增在宫颈病变中的表达状态并探讨其临床意义。方法:对89例宫颈病变患者及20例正常宫颈组织者进行宫颈脱落细胞的人乳头瘤病毒基因型检测,另采用间接免疫荧光染色技术检测中心体。结果:(1)在正常宫颈组织细胞内可见大小及数目正常的中心体,而在CIN及宫颈鳞癌组织中则发现不同程度的异常中心体,表现为中心体形态异常及数目增多。(2)在CINⅠ、CINⅡ及CINⅢ组中心体异常率逐渐增加,差异有统计学意义(P=0.000)。宫颈鳞癌Ⅰ期、Ⅱ期、Ⅲ期肿瘤细胞中心体异常率逐渐增加,差异有统计学意义(P=0.005)。(3)CIN组与宫颈鳞癌组细胞中心体异常细胞率分别为(1.76±1.74)%、(4.46±2.08)%,差异有统计学意义(P=0.000)。中心体异常细胞率随CIN级别和宫颈鳞癌临床分期的增加而增加,两者呈正相关(P=0.000)。结论:(1)中心体异常扩增是宫颈鳞癌细胞恶性表型的明显特征之一,有望发展成为宫颈鳞癌辅助诊断的标志物。(2)中心体异常扩增是宫颈鳞癌发生发展过程中的早期事件,可能与宫颈鳞癌发展进程有关。Objective: To investigate the centrosome abnormalities in cervical lesions and its clinical significance. Methods: 89 cases of cervical lesions and another 20 cases of normal cervical tissue were tested HPV-DNA by Hybri Max. Besides,Cervical tissue specimens were investigated for centrosome status by using indirect immunofluorescence. Results:( 1) Normal cervical epithelium showed the centrosome with normal number and size,while CIN and cervical squamous cell cancer were found in varying degrees of centrosome abnormalities,centrosome abnormalities showed abnormal centrosome morphology or number increased.( 2) The proportion of abnormal centrosome of CINI,CINII,CINIII increased gradually,the difference was statistically significant. The proportion of abnormal centrosome of the stage Ⅰ,Ⅱ,Ⅲ of the cervical squamous cell carcinoma increased gradually,the difference was statistically significant.( 3) The proportion of abnormal centrosome of CIN and cervical squamous cell carcinoma respectively( 1. 76 ± 1. 74) % and( 4. 46 ± 2. 08) %,the difference was statistically significant. Abnormal centrosome rate increased gradually with the increase of CIN grade and clinical stage of cervical squamous cell cancer and the two showed a significant positive correlation. Conclusions:( 1)Centrosome abnormalities is one of the characteristics of cervical squamous cell cancer and it can serve as a complementary biomarker to cytological examination.( 2) Centrosome amplification was an early event and that might play a role in the progress of cervical squamous cell cancer.
