平板运动试验时QT间期离散度对冠心病的诊断价值

摘　　要：目的:探讨平板运动试验时QT间期离散度对冠心病的诊断价值。方法:选取90例疑诊为冠心病的患者,平板运动试验后1月内行冠状动脉造影术,根据其结果分为冠心病组63例和非冠心病组27例,测量两组患者平板运动试验前、中、后12导联心电图的QT间期,将QT间期离散度≥50 ms作为冠心病的诊断标准。结果:冠心病组QT间期离散度平板运动试验前、后与平板运动试验中比较,差异有统计学意义(P<0.05),平板运动试验前与平板运动试验后比较,差异无统计学意义(P>0.05);非冠心病组平板运动试验前、中、后QT间期离散度差异无统计学意义(P>0.05)。QT间期离散度≥50 ms诊断冠心病的灵敏度为88.89%,特异度为92.59%。结论:平板运动试验时QT间期离散度≥50 ms作为冠心病的诊断标准具有重要的临床价值。Objective： To investigate the value of QT interval dispersion in treadmill exercise test in the diagnosis of coronary heart disease. Methods： 90 cases with suspected coronary heart disease from October,2012 to September,2014 in our hospital were selected,coronary angiography was carried out after one month treadmill exercise test,according to the results,the patients were divided into coronary heart disease group（ 63 cases） and non-coronary heart disease group（ 27cases）,the 12-lead ECG QT interval was measured before,during and after the treadmill exercise test,and the QT interval dispersion ≥ 50 ms was as the diagnostic criterion for coronary heart disease. Results： In coronary heart disease group,the QT interval dispersion before and after treadmill exercise test compared with treadmill exercise test,the difference was statistically significant（ P〈0. 05）,before treadmill exercise test compared with after treadmill exercise test,the difference was not statistically significant（ P〉0. 05）; In non-coronary heart disease group,the QT interval dispersion before,during and after the treadmill exercise test had no significant difference（ P〉0. 05）. The sensitivity of QT interval dispersion ≥ 50 ms in the diagnosis of coronary heart disease was 88. 89%,and the specificity was 92. 59%.Conclusion： As the diagnostic criterion for coronary heart disease,the QT interval dispersion ≥ 50 ms in treadmill exercise test has important clinical value.
