医用胶在双侧腹股沟疝行腹腔镜完全腹膜外疝修补术中的应用

摘　　要：目的探讨医用胶在双侧腹股沟疝行腹腔镜完全腹膜外疝修补术（totally extraperitoneal prosthesis，TEP）中的应用效果。方法对40例双侧8复股沟疝患者行TEP术，术中充分游离8复膜前间隙，精索骨骼化，平铺补片，用医用胶粘合固定。结果40例患者中，1例中转为经腹腔腹膜前修补术。手术时间95～130min，平均（105．0±20．2）min；术后住院时间2～5d，平均（3．0±0．8）d。术后并发腹股沟区血肿1例，皮下气肿2例，阴囊积液3例，附睾炎1例；随访5～48个月，平均（22．0±3．6）个月，无复发病例。结论医用胶在双侧腹股沟疝行腹腔镜完全腹膜外腹股沟疝修补术中应用安全有效。Objective To investigate the clinical efficacy of medical glue in totally extraperito-neal bilateral laparoscopic inguinal hernia repair (TEP).Methods Forty patients with bilateral inguinal hernias received TEP.The preperitoneal space was dissociated and spermatic cord was skeletonized.The patch was fixed with medical glue.Results One patient was converted to transabdominal preperitoneal herniorrhaphy.The average operation time was (105.0±20.2)mi-nutes (range,95 to 130 minutes).The average length of postoperative hospital stay was (3.0± 0.3)days (range,2 to 5 days).After operation,1 patient had inguinal hematoma,2 patients had subcutaneous emphysema,3 patients had scrotal effusion,and 1 patient had epididymitis.No re-currence was found during an average follow-up of (22.0±3.6)months (range,5 to 48 months). Conclusion The medical glue can be safely and effectively used in TEP.
