^(18)F-FDG符合线路显像联合肿瘤标志物检测对腹水的诊断价值

摘　　要：目的评价18F-FDG符合线路显像联合血清CEA、AFP、CA125、CA199检测在不明原因腹腔积液鉴别诊断中的价值。方法回顾性分析2010年12月至2013年6月因腹水原因待查行18F-FDG符合线路显像的患者51例,用18F-FDG符合线路、肿瘤标志物以及18F-FDG符合线路联合肿瘤标记物三种方法对腹水的性质进行鉴别诊断。方法间的诊断效能用配对四格表的χ2检验进行统计学分析。结果18F-FDG符合线路显像对恶性腹水的诊断敏感性为83.33%,特异性为33.33%,准确度为68.6%。肿瘤标志物对恶性腹水的诊断敏感性为86.11%,特异性为13.33%,准确度为64.71%。18F-FDG符合线路显像联合肿瘤标记物诊断敏感性为88.89%,特异性为100%,准确性为92.16%,与单纯18F-FDG符合线路显像(χ2=4.08,P<0.05)及肿瘤标记物检测(χ2=5.5,P<0.05)比较,差异有统计学意义。结论18F-FDG符合线路显像联合肿瘤标记物检测对腹水性质的诊断准确性优于单纯18F-FDG符合线路显像及肿瘤标记物检测。Objective To evaluate the value of detection of tumor makers combined with18F-FDG dual head coincidence imaging in differential diagnosis of agnogenic ascites. Methods The data of 51 patients with agnogenic ascites who received18F-FDG dual head coincidence imaging from December 2010 to June 2013 were retrospectively analyzed. These patients were differentially diagnosed for the nature of ascites by18F-FDG dual head coincidence imaging only or tumor makers only,and18F-FDG dual head coincidence imaging combined with detection of tumor maker. The results were statistically analyzed by Chi-square test. Results The diagnosis sensitivity,specificity and accuracy of18F-FDG dual head coincidence imaging in diagnosis of malignant ascites were 83. 33%,33. 33%,68. 6%,respectively,which in detection of tumor makers were 86. 11%,13. 33%,64. 71%,respectively,however,which in18F-FDG dual head coincidence imaging combined with detection of tumor makers were 88. 89%,100%,92. 16%,respectively. There were significant differences in diagnosis specificity and accuracy for malignant ascites between18F-FDG dual head coincidence imaging combined with detection of tumor makers and18F-FDG dual head coincidence imaging only（ χ2= 4. 08,P 〈0. 05） or detection of tumor makers only（ χ2= 5. 5,P 〈0. 05）. Conclusion18F-FDG dual head coincidence imaging combined with detection of tumor makers is superior to simple18F-FDG dual head coincidence imaging or simple detection of tumor makers in diagnostic accuracy for the nature of malignant ascites.
