从PI3K/AKT-trpv1信号通路介导“二次打击”探讨清胰汤防治急性胰腺炎的临床研究

摘　　要：目的通过观察急性胰腺炎治疗前、后临床症状、血PI3K/AKT-trpv1信号通路中相关指标表达,探讨清胰汤用于防治急性胰腺炎的临床研究。方法 30例急性胰腺炎患者符合纳入标准、诊断标准及排除标准,按就诊顺序1∶1随机分为两组。治疗组:基础治疗+清胰汤;对照组:基础治疗,连续治疗3周。详细记录治疗前、治疗后1周、2周及3周临床症状积分;应用荧光定量PCR法检测血P110、P85、AKT、trpv1指标表达。结果治疗组总有效率为73.33%,病死率为6.67%;对照组总有效率为60%,病死率为13.33%,经比较有统计学差异(P<0.05)。治疗前,血P110、P85、AKT、trpv1表达呈上升趋势;治疗后,治疗组血P110、P85、AKT、trpv1表达均明显下降,较治疗前有统计学差异(P<0.05)。对照组血P110、P85、AKT、trpv1表达较治疗前无明显变化。结论清胰汤可能通过调控PI3K/AKT-trpv1信号通路相关指标表达,发挥协同缓解急性胰腺炎的效用。Objective We aim to assess the effect of Qingyi Decoction in prevention and treatment of acute pancrea- titis through observing the changes of the clinical symptoms before and after treatment and the related indices of PI3K/ AKT -trpvl signaling pathway. Methods According to the order of treatment,30 cases of acute pancreatitis patients who must met the inclusion criteria, diagnostic criteria and exclusion criteria were randomly divided into 2 groups of 1： 1. The treatment group was used foundation treatment with Qingyi decoction for 3 weeks; and the control group was used foundation treatment for 3 weeks. Recording symptoms integration before treatment, then after 1 weeks, 2 weeks and 3 weeks treatment. Using real -time PCR to detect the changing indexs of blood Pll0, P85, AKT, TRPV1. Results The total effective rate of treatment group was 73.33%, the fatality rate was 6. 67%. The total effective rate of control group was 60%, the fatality rate was 13.33%. There is a statistical difference by comparison. Before treatment, the ex- pression of P110, P85, AKT and TRPV1 was on the rise. After treatment, the expression of P110, 1＇85, AKT, TRPV1 were significantly decreased. There is a statistical difference. The expression of P1 !0 , P85, AKT and TRPVI in the con- trol group have no obvious changes before treatment. Conclusions Qingyi decoction can alleviate acute pancreatitis by regulating the related indexes of PI3K/AKT - trpvl signaling pathway.
