韩氏穴位神经刺激仪对剖宫产术后慢性疼痛的影响

摘　　要：目的:评估围术期韩氏穴位神经刺激仪(Han's acupoint nerve stimulator,HANS)对剖宫产术后慢性疼痛的疗效。方法:177例择期剖宫产妇女随机分入HANS组(n=84)和对照组(n=93)。HANS组行围术期HANS治疗,对照组未予电刺激。采用视觉模拟评分(Visual analogue scale,VAS)评估术后24 h、48 h、72 h疼痛程度,测定术前、术后24 h、72 h产妇血清白细胞介素-6(interleukin-6,IL-6)、白细胞介素-10(interleukin-10,IL-10)浓度,追踪术后3个月、6个月慢性疼痛。结果:HANS组产妇术后72 h VAS较对照组低(P<0.05);两组产妇术后IL-6均较术前增高,术后72 h HANS组产妇IL-6较对照组低,而IL-10较对照组高(P<0.05);术后3个月HANS组术后慢性疼痛发生率较对照组低(P<0.05)。结论:HANS有助于缓解剖宫产术后早期疼痛并调节血清IL-6、IL-10水平,减少剖宫产术后慢性疼痛。Objective： To evaluate the effects of Han＇s acupoint nerve stimulator （HANS） on chronic postsurgical pain （CPSP） following caesarean section. Methods： 177 women undergoing elective caesarean section were randomly assigned to receive perioperative HANS （HANS group, n=84） or no stimulation （control group, n=93）. Postoperative pain was evaluated by visual analogue scale （VAS） at 24, 48 and 72 hours after surgery, respectively. Blood samples were taken before surgery, 24 and 72 hours after surgery to detect the serum levels of interleukin-6 （IL-6） and interleukin-10 （IL-10）. The prevalence of CPSP was followed up at 3 and 6 months after caesarean section. Results： The VAS in HANS group were lower than those in control group at 72 hours after surgery （P 〈 0.05）. The levels of IL-6 in two groups increased postoperatively. The levels of IL-6 in HANS group were lower than those in control group while the levels of IL-10 in HANS group were higher than those in control group at postoperative 72 hours （P 〈 0.05）. The incidence and degree of CPSP in HANS group were lower at postoperative 3 months as compared with control group （P 〈 0.05）. Conclusion： HANS therapy could reduce the severity of pain and regulate the serum levels of IL-6 and IL-10 at early stage of postoperation, and is effective for CPSP following caesarean section.
