纵隔神经内分泌癌的影像学表现

摘　　要：目的分析纵隔神经内分泌癌的影像学特点及临床应用。方法回顾性分析经病理证实的5例纵隔神经内分泌癌的CT、MRI以及PET/CT征象,以便对肿瘤临床分期以及制定治疗方案提供信息。结果 5例纵隔神经内分泌癌分别位于前纵隔2例、中纵隔2例、后纵隔1例。其中4例呈浸润性生长、包绕周围血管,1例位于隆突下呈膨胀性生长;4例CT检查呈中度强化。1例在MRI上呈不均匀性较长T1较长T2信号,3例行18F-PET-CT检查,最大摄取值中位数（SUVmax）为9.2（6.7~28.5）。本组5个病例全部发生远处转移,肿瘤分期均为Ⅳ期,并接受放化疗。结论纵隔神经内分泌癌呈浸润性生长、不均匀软组织密度或信号、中度对比增强、易转移、糖代谢明显增高等特点,影像检查有利于肿瘤分期和制订治疗方案。Objective To analyze the imaging characteristics of mediastinal neuroendocrine carcinoma and their clinical ap- plication. Methods In 5 patients, the imaging findings of mediastinal neuroendocrine carcinoma confirmed by pathology on CT, MR and PET/CT were analyzed retrospectively to perform the clinical staging and determine the therapeutic plans. Results Of total 5 ca- ses of mediastinal neuroendocrine carcinoma, 2 were located in anterior mediastinum, 2 in middle mediastinum and 1 in posterior medi- astinum. The lesions presented infiltrative growth wrapping around the blood vessel in 4 cases and expansive growth at the subcarinal in 1 patients. The lesions showed moderate enhancement on contrast CT in 4 cases and heterogeneous long T1 and long T2 signal on MRI in 1 case. The media of maximal standard uptake value （SUVmax） was 9.2（6.7 -28.5 ） on ~SF-FDG PET/CT. In all 5 cases, the metastases were found that were defined as stage 1V and chemoradiation were performed. Conclusion Mediastinal neuroendocrine car- cinoma presented infiltrative growth, heterogeneous density or signal intensity, moderate enhancement, easy metastasis, and higher gly- cometabolism, which was in favour of neoplasm staging and determination of treatment protocol.
