摘  要:目的:对盐酸去甲乌药碱(HG)注射液,作为心脏负荷试验药物用于核素心肌灌注显像(MPI)诊断冠心病的有效性,进行临床观察,并与腺苷注射液进行比较,以冠状动脉造影结果为金标准对照,以验证其在冠心病诊断中的有效性。方法:采取随机、开放、阳性药对照(腺苷注射液)、交叉设计(2×2)及临床研究方法。入选病例60例,男性36例,女性24例,平均年龄(56.1±13.8)岁,分别将HG注射液及腺苷注射液,作为心脏负荷试验药物用于MPI诊断冠心病的结果,与冠状动脉造影的结果相比较,描述其诊断冠心病的灵敏度和特异度。结果:以病例为基础的MPI诊断结果,去甲乌药碱和腺苷诊断冠心病敏感性分别为69.0%,65.5%;特异性76.9%,76.0%准确性75.0%,73.3%;两组间差异无统计学意义(P>0.05)。结论:HG作为心脏负荷试验药物,用于MPI诊断冠心病,与腺苷注射液诊断效能相当。Objective:To investigate the clinical availability of hydrochloric acid higenamine injection as the medicine of cardiac stress test versus adenosine injection by using radionuclide myocardial perfusion ima- ging (MPI) to diagnose coronary artery disease( CAD), which used coronary angiography (CAG) as the gold standard for this study. Methods :The study adopted randomized, open, positive control (adenosine injection) and a cross-over design as the clinical research methods. 60 cases were selected in this study ,36 male, 24 fe- male; mean age(56. 1±13. 8)years, hydrochloric acid higenamine injection and adenosine injection respec- tively as the medicine of cardiac stress test were performed by using radionuelide MPI to diagnose CAD, and the results were compared to those generated using CAG. The sensitivity and specificity of diagnosing CAD using the two different drugs was determined in the last statistical analysis. Results :On the basis of MPIs results anal- ysis of inclusive cases, the diagnosis values of hydrochloric acid higenamine injection and adenosine injection u- singt3test were as followed: SEN 69. 0%, 65. 5% (X^2 =0. 078, P 〉0. 05), SPE 80. 6%, 80. 6% (X^2 =0. 0, P 〉 0. 05 ), ACC 75.0% , 73,33%. There were no obvious statistical difference for each value between two in- jections. Conclusion:Higenamine as a drug for cardiac stress test, having similar value with adenosine in diag- nosis of CAD.
