舒芬太尼联合硬膜外麻醉在妇科腹腔镜手术中的应用价值研究

摘　　要：目的：分析舒芬太尼联合硬膜外麻醉在妇科腹腔镜手术中的应用价值。方法：选择于2012年10月~2013年10月间接受腹腔镜手术治疗的妇科患者276例作为研究对象,随机分为舒芬太尼联合硬膜外麻醉的观察组及单纯硬膜外麻醉的对照组患者,检测两组患者的硬膜外麻醉效果、防治内脏牵拉效果及应激反应指标情况。结果：（1）硬膜外麻醉完成后各个时间点,观察组患者的VAS评分均明显低于对照组患者,且BCS评分明显高于对照组患者（P〈0.05）;（2）硬膜外麻醉完成后,观察组患者的防治内脏牵拉总有效率明显高于对照组患者（P〈0.05）;（3）麻醉后,两组患者的应激水平均高于麻醉前（P〈0.05）,且观察组患者的ANP、Cor、ACTH水平均低于对照组（P〈0.05）。结论：舒芬太尼联合硬膜外麻醉可以有效减少患者术中疼痛并提高舒适度,显著防治术中内脏牵拉效果,同时降低患者的应激水平。Objective： To analyze the effect of sufentanil combined with epidural anesthesia on gynecological patients under laparoscopic surgery.Methods：Choosed patients with laparoscopic gynecological surgery between October 2012 to October 2013 as observe object,randomly divided into observer group with sufentanil combined epidural anesthesia and control group with epidural anesthesia only,detected the epidural anesthesia effect,control visceral effects and stress indicators.Results：（1）After epidural anesthesia,observe group patients＇ VAS scores were significantly lower than control group,at various time point,and BCS score was significantly higher than control group（P〈0.05）;（2） After epidural anesthesia,the control visceral effects of observation group patients was significantly higher han control group（P〈0.05）;（3） After anesthesia,two groups stress indicators level was higher than before（P〈0.05）,and observation group patients＇ ANP,Cor,ACTH levels were lower than control group（P〈0.05）.Conclusion：Sufentanil combined with epidural anesthesia surgery can effectively reduce pain and improve patient comfort,significant prevention of intraoperative visceral effect,while reducing the stress level of the patient.
