急性高容性血液稀释联合控制性降压在人工全髋关节置换术中的应用

摘　　要：目的：观察急性高容性血液稀释联合控制性降压在人工全髋关节置换术中血液保护的临床效果。方法：选择本院2013年7~12月收治的60例需行人工全髋关节置换术患者60例作为研究对象,按照数字随机法分为两组,每组30例,观察组采取急性高容性血液稀释联合控制性降压技术,对照组单纯采取控制性降压技术,观察两组患者术中输血量及血红蛋白水平、红细胞压积变化。结果：观察组术中出血量（746.4±213.4）ml与对照组（729.8±224.2）ml比较无明显差异,P〉0.05;观察组输血量（123.1±56.4）明显低于对照组（298.9±65.4）ml,P〈0.05;术后观察组Hb、HCT分别为（112.9±10.9）g/L、（31.4±5.4）%,均高于对照组,均P〈0.05;观察组术后BUN、Cr与对照组比较均无明显差异,均P〉0.05。结论：急性高容性血液稀释联合控制性降压在人工全髋关节置换术可有效减少出血,且较为安全。Objective： To observe the effect of acute hypervolemic blood combined with controlled hypotension blood conservation in total hip arthroplasty for blood conservation.Methods：Selected 60 cases of patients with total hip replacement treated in the hospital from 2013 July-2013 year in December the research object,They were divided into two groups according to random number table with 30 cases in each group,the observation group adopted acute hypervolemic blood hypotension combined with controlled hypotension blood conservation,the control group only took controlled hypotension blood conservation,observe the change of hemoglobin leve and hematocri between two groups.Results：The amount of intraoperative bleeding was（746.4±213.4）ml in the observation group and the control group was（729.8±224.2）ml and there were no significant difference,P〉0.05;But the blood transfusion group in the observation group was（123.1±56.4） and significantly lower than that of the control group＇（298.9±65.4）ml,P〈0.05;Hb,HCT were（112.9±10.9）g/L,（31.4±5.4）% in the observation group and higher than the control group after surgery,P〈0.05;BUN,Cr compared with the control group had no significant difference,P〉0.05.Conclusion：Acute hypervolemic hemodilution combined with controlled hypotension in the total hip replacement can effectively reduce bleeding,and more safe.
