摘  要:【目的】探讨新兵缺血预适应后血管反应性与高原训练损伤的相关性。【方法】48名初次进入高原的男性新兵,在平均海拔3 300 m的高原拉练8 d后返回驻地,返回驻地第3天采血查血生化及各项生理指标。进行缺血预适应干预,用血压计袖带给予右上臂5 min的加压与放气,共重复3个循环。测量缺血预适应后第1、3、5、10、15及20 min的双臂血压。20 d后再登高原,登高原3 d后采血查血生化及各项生理指标。【结果】肌酸激酶(creatine kinase,CK)增高幅度与缺血手臂缺血预适应后1 min和5 min的平均收缩压下降幅度呈负相关(r=-0.391,P=0.006),与缺血手臂缺血预适应后1 min和5 min的平均舒张压下降幅度呈负相关(r=-0.281,P=0.048),天门冬氨酸转移酶(aspartate transferase,AST)增高幅度与缺血手臂缺血预适应后1 min和5 min的平均收缩压下降幅度呈负相关(r=-0.319,P=0.027)。【结论】上臂缺血预适应后收缩压和舒张压的降低幅度与高原训练肌肉损伤的程度负相关,提示缺血预适应后血管反应性的高低可作为一种预测高原训练损伤程度的筛查手段。[ Objective ] To explore the correlation between ischemic preconditioning-induced vascular reactivity and altitude training injury in recruits. [ Method ] Forty-eight male recruits with no experience of living in the plateau area were enrolled in this studyl They ) received an 8-day Plateau Armed Field Training program in the plateau with an average height of 3 300 m. Blood samples were collect- ed 3 days later when they returned to the plain area to measure blood biochemical and other physiological indexes. Ischemie precondi- tioning was induced by pressing right upper arm for 5 min and deflating for 5 rain with blood-pressure meter in 3 cycles/Then blood pressures of both arms were measured at 1st, 3rd, 5th, 10th, 15th and 20th min after the ischemic preconditioning. After 20 days, they were sent to the plateau area again and blood samples were collected at the fourth day. [ Results ] The increase of creatine kinase (CK) was negatively correlated with the decrease of mean systolic and diastolic blood pressures at 1st and 5th min after the ischemic precon- ditioning (r = -0.391, P - 0.006; r = -0.281, P = 0.045), respectively. Whereas, the increase of aspartate transferase (AST) was nega- tively correlated with the decrease of mean systolic blood pressure at 1st and 5th min after the ischemic preconditioning (r = -0.319, P = 0.027). [Conclusions] The decrease of systolic and diastolic blood pressures induced by upper-arm ischemic preconditioning was negatively correlated with the altitude training muscle damage level. Thus, arm ischemic preconditioning-induced vascular reactivity might be used as a simple and effective approach to predict the altitude training injury.
