摘 要:目的 探讨男性2型糖尿病病人骨密度的变化及其影响因素。方法 采用美国QDR 4500W型双能量X线骨密度仪测定66例男性2型糖尿病病人和53例男性健康体检者第1~4腰椎(L1、L2、L3、L4、L1~4总体)及左侧髋部(股骨颈、大转子、Ward三角、股骨干、左髂骨总体)骨密度,同时记录受试者年龄、病程,测定身高、体质量、腰围、臀围、糖化血红蛋白(HbA1c)、空腹C肽、餐后2hC肽等指标,多元回归分析男性2型糖尿病病人骨密度的影响因素。结果 两组各部位骨密度比较差异均无显著性(P〉0.05)。多元回归分析显示,男性2型糖尿病病人L1~4总体和左髂骨总体骨密度与HbA1c呈负相关(β=-0.302、-0.256,P〈0.05)。结论 男性2型糖尿病病人骨密度可以是正常的,并且与HbA1c关系密切。Objective To explore the bone mineral density (BMD) and its influencing factors in male patients with type 2 diabetes mellitus (T2DM). Methods Using dual-energy X-ray absorptiometry (DXA), 66 male patients with T2MD and 53 healthy male controls were detected for their bone density as follows= lumbar spine 1-4 and left hip-including femur neck,greater trochanter,Ward trlangle,shaft of femur, and left ilium. The patient age, course of disease, height, weight, waistline, hip circum- ference, serum HbAlc, fasting C petide (FCP), and 2-hour postprandial C-petide were recorded. Influencing factors of bone densi- ty of the patients were analyzed by employing multivariate regression analysis. Results The differences of BMD between the two groups were not significant (P〉0.05). Multivariate regression analysis showed the BMD of L1-4 and left ilium in the patients was negatively correlated with HbA1c (β= -0.302,-0.256;P〈0.05). Conclusion Bone mineral density of male patients with type 2 diabetes mellitus can be normal and closely associated with HbA1e.
