摘 要:目的 探讨冠心病(CHD)病人血清胎球蛋白A(Fetuin-A)、白细胞介素-35(IL-35)浓度的变化及其应用价值,并分析其与α羟丁酸脱氢酶(HBDH)和乳酸脱氢酶(LDH)的相关性。方法 采用酶联免疫吸附法测定60例不同类型CHD病人血清Fetuin-A、IL-35浓度,同时检测CHD病人血清LDH、HBDH浓度,并与20例健康体检者(对照组)进行比较。结果 CHD组血清Fetuin-A、IL-35浓度显著低于对照组(t=3.60、3.34,P〈0.05)。急性心肌梗死(AMI)组和不稳定型心绞痛(UAP)组血清Fetuin-A、IL-35浓度显著低于稳定型心绞痛(SAP)组和对照组(F=4.28、8.77,q=3.54~77.28,P〈0.05)。CHD组血清HBDH、LDH浓度均高于对照组(t=2.21、2.20,P〈0.05)。AMI组血清HBDH、LDH浓度显著高于UAP组、SAP组及对照组(F=8.56、8.72,q=6.01~7.07,P〈0.01)。CHD病人血清Fetuin-A与IL-35浓度呈正相关(R^2=0.870,P〈0.01);Fetuin-A与HBDH、LDH浓度均呈负相关(R^2=0.034、0.028,P〈0.1)。CHD组血清LDH、HBDH、Fetuin-A、IL-35浓度变化与对照组相比的AUCROC为0.602~0.746,AMI组为0.816~0.915,UAP组为0.544~0.815。结论 血清Fetuin-A和IL-35浓度变化对CHD有较高的诊断价值;在对不同类型CHD病人的诊断中,除IL-35对UAP病人价值较低外,两者均有较高的诊断价值。Objective To investigate the changes of serum Fetuin-A and interleukin-35 (IL-35) levels and their correla- tion with hydroxybutyrate dehydrogenase (HBDH) and lactic dehydrogenase (LDH) in patients with coronary heart disease (CHD). Methods Employing enzymeqinked immune sorbent assay, serum Fetuin-A, and IL-35 were detected in 60 patients with different-type CHD, and, at the same time, serum LDH and HBDH were measured as well. The results were compared with 20 healthy controls. Results Serum levels of Fetuin-A and IL-35 in CHD patients were notably lower than that in the controls (t = 3.60,3.34 ; P 〈0.05), that in acute myocardial infarction (AMI) and unstable angina pectoris (UAP) were lower than that stable angina pectoris (SAP) (F = 4.28,8.77 ; q = 3.54 - 77.28 ; P 〈 0.05 ). Serum LDH and HBDH in CHD were higher than that in the control group (t = 2.21,2.20;P〈0.05) and that in AMI were higher than that in U AP, SAP and control groups (F : 8.56, 8.72;q:6.01-7.07;P〈0.01). In CHD, Fetuin-A was positively correlated with IL-35 (R2=0.870,P〈0.01), and Fetuin-A was negatively correlated with both HBDH and LDH (R 2 : 0.034,0.028;P d0.1). Compared with the control group, the area under the receiver operating characteristic curves (AUC^ROC) of LDH, HBDH, Fetuin-A and IL-35 were 0. 602- 0. 746 in CHD patients; AUC^ROC of AMI patients were 0.816-0.915; AUC^ROC of UAP patients were 0.544--0.815. Conclusion The changes of serum levels of Fetuin-A and IL-35 have a greater value in the diagnosis of coronary heart disease, except IL-35 for unstable angina pectoris.
