摘 要:目的 探讨冠心病(CHD)危险因素在不同年龄组病人中的分布及聚集状况,为不同年龄组人群早期预防CHD发生发展提供依据。方法 2012年10月—2013年5月,青岛大学附属医院心血管内科临床诊断或疑诊CHD并行冠状动脉造影检查病人828例,按年龄分为青年组、中年组、老年组,各年龄组病人再根据造影结果分为CHD组和对照组。分析各年龄组CHD组与对照组的临床资料及危险因素聚集情况,比较不同年龄组CHD病人危险因素分布的差异。结果 在男性、吸烟、高血压(HBP)、糖尿病(DM)比例及高密度脂蛋白(HDL)、体质量指数(BMI)、纤维蛋白原(Fbg)水平上,CHD组与对照组差异有统计学意义(χ^2=4.210~45.304,t=2.139~5.875,P〈0.05)。CHD组与对照组可控制的5种传统危险因素聚集状况差异有显著性,CHD组中有危险因素聚集者比例明显高于无危险因素聚集者(χ^2=13.171-30.529,P〈0.01)。不同年龄组CHD病人的传统危险因素分布情况存在差异,在男性、吸烟比例及BMI、三酰甘油(TG)水平方面青年组高于中年组、老年组,在HBP、DM比例及HDL水平方面中年组、老年组高于青年组(F=6.949-9.901,χ^2=13.795-56.486,P〈0.01)。结论 CHD的传统危险因素存在聚集现象,不同年龄组危险因素分布存在差异。Objective To investigate the distribution and aggregation of risk factors of coronary heart disease (CHD) in patients of different age groups, and provide evidence for their early prevention of the occurrence and development of this disease. Methods During October 2012 May 2013 period, 828 patients diagnosed or suspected as having CHD underwent coronary arte- riongraphy were divided into youth group, middle-age group and older-age group, and they were again divided into CHD group and control group according to the result of angiography. The clinical data and risk factors of patients in the two groups were analyzed, and the difference of distribution of risk factors in different-age--group CHD patients was compared. Results In male, the diffe- rences between CHD group and control group were significant with regard to proportion of smoking, hypertension, diabetes and the level of high density lipoproteins (HDL), body mass index (BMI), and plasma fibrinogen (Fbg) (x^2z =4.210-45.304,t =2.139-5.875, P〈0.05). The aggregation of five controllable traditional coronary risk factors was significantly higher in CHD groups than that in the control (x^2 =13.171-30.529,P〈0.01). In CHD group, the proportion of aggregation in patients with risk factors was significantly higher than those without. The distribution of traditional risk factors was different in different age groups. In male, the proportion of smoking, BMI, TG in the youth group was higher than that in the middle-age and older age groups, and in terms of HBP, DM and HDL, which were higher in the middle-age and older age group (F=6.949-9.901 ,x^2 = 13.795-56.486 ,P〈0.01). Conclusion There is an aggregation of traditional coronary risk factors in coronary heart disease, and the distribution of risk factors in different age groups is different.
