医院社区联动护理管理模式在肿瘤化疗患者PICC管理中的应用

摘　　要：目的：探讨医院社区联动护理管理模式对肿瘤化疗患者PICC留置期间并发症和健康状况的影响。方法2013年6月—2014年6月将126例行化疗方案且置入PICC的肿瘤患者按入院先后顺序分为对照组和观察组，各63例。对照组患者给予常规护理，观察组患者在此基础上实施社区联动护理管理模式。比较两组患者的PICC导管相关并发症发生情况和置管期间的健康状况，其中健康状况采用健康状况问卷（SF-36）进行评价。结果实施社区联动护理管理模式后，观察组患者的穿刺点持续渗血、穿刺点感染、静脉炎发生率均低于对照组，并发症总发生率为11.11%，低于对照组的53.97%，差异有统计学意义（P〈0.05）。观察组患者健康状况的各项评分均优于对照组，差异有统计学意义（P〈0．05）。结论社区联动护理管理模式能够提升社区医院的护理质量，改善肿瘤化疗患者的生存质量，对整合医疗资源，保障患者利益起到积极的作用。Objective To explore the influence of the hospital-community integrated nursing on PICC management of tumor patients undergoing chemotherapy. Methods A total of 126 patients were randomly divided into control group and experimental group on average from June 2013 to June 2014, and the patients of control group used routine care while the patients of experimental group adopted hospital-community integrated nursing. We compared the incidence of PICC complications and health status, in which health status assessed by SF-36. Results After implementation of hospital-community integrated nursing, the incidence of durative staxis, infection and phlebitis in the experimental group were lower than those of the control group, and the total complication incidences （11. 11%） were lower than 53. 97% of the control group （P〈0. 05）. Each scores of health status items in the experimental group was better than that of the control group as well （P 〈0. 05）. Conclusions The hospital-community integrated nursing can increase the nursing quality of community hospital and the quality of life of patients, and play a positive role on integrating medical resources and protect the benefits of patients.
