摘 要:目的:研究尿液巨细胞病毒DNA(CMV-DNA)联合血清巨细胞病毒特异抗体(CMV-IgM)检测对婴儿巨细胞病毒(CMV)感染的诊断价值。方法:选取2012年8月~2014年8月聊城市人民医院疑似CMV感染的婴幼儿70例,应用化学发光法检测血清CMV-IgM,采用实时荧光定量PCR法(FQ-PCR)检测尿液CMV-DNA,比较抗病毒治疗前后的检测结果。结果:单独检测尿CMV-DNA的阳性率为62.86%(44/70),单独检测血清CMV-IgM的阳性率为55.71%(39/70),两者比较差异无统计学意义(P〉0.05);两者联合检测的阳性率为77.14%(54/70),与两种方法单独检测比较差异均有统计学意义(P〈0.05);抗病毒治疗后两种检测结果均显著降低,与治疗前比较差异有统计学意义(P〈0.05)。结论:尿液CMVDNA联合血清CMV-IgM检测能显著提高婴儿CMV感染的阳性率。Objective: To research the value of urine cytomegalovirus- DNA( CMV- DNA) detection combined with serum cytomegalovirus- IgM( CMV- IgM) detection in diagnosis of cytomegalovirus( CMV) infection. Methods: Seventy infants suspected of CMV infection were selected from the hospital from August 2012 to August 2014,chemiluminescence method was used to detect serum CMV-IgM,real- time fluorescent quantitative- PCR( FQ- PCR) was used to detect urine CMV- DNA,the detection results before and after antiviral treatment were compared. Results: The positive rate of urine CMV- DNA detection was 62. 86%( 44 /70),the positive rate of serum CMV- IgM detection was 55. 71%( 39 /70),there was no statistically significant difference( P〈0. 05); the positive rate of joint detection was 77. 14%( 54 /70),compared with single detection,there were statistically significant differences( P〈0. 05); after antiviral treatment,the detection results of the two methods decreased significantly,compared before treatment,there were statistically significant differences( P〈0. 05). Conclusion: Urine CMV- DNA detection combined with serum CMV- IgM detection can significantly improve the positive detection rate of cytomegalovirus infection among infants.
