摘  要:目的:探讨影响婴儿瞬态诱发耳声发射(Transient Evoked Otoacoustic Emission,TEOAE)听力筛查通过率的相关因素。方法:选取2012年7月~2013年12月在该单位听力筛查中心进行TEOAE首次筛查的婴儿947人为研究对象。采用自制调查问卷,对婴儿的家庭情况、分娩时情况、孕期情况及新生儿情况等因素进行调查,分析影响婴儿听力筛查通过率的相关因素。结果:在947名婴儿中,TEOAE初筛未通过率(包括双耳未通过和单耳未通过)为11.8%,单因素分析结果显示,出生体重、胎龄、胎数、耳廓或外耳道畸形、生后是否给氧、是否住进NICU、父母亲文化程度和家庭收入,差异有统计学意义(P〈0.05);多因素分析结果显示,胎数、耳廓或外耳道畸形及生后是否给氧差异有统计学意义(P〈0.05或P〈0.01)。结论:胎数、外耳畸形及生后是否给氧是TEOAE筛查未通过的影响因素,应将双胎或多胎、外耳道或耳廓畸形及生后给氧的新生儿列为重点筛查对象,密切随访和及早干预,避免听力损失对儿童发育的影响。Objective: To explore the related factors of passing rate of transient evoked otoacoustic emission( TEOAE) for hearing screening of infants. Methods: A total of 947 infants receiving TEOAE screening for the first time in the hospital from July 2012 to December2013 were selected,a self- designed questionnaire was used to survey the family condition,delivery condition,pregnancy condition,and neonatal condition; the related factors of passing rate of neonatal hearing screening were analyzed. Results: Among 947 infants,the failing rate of TEOAE primary screening( including the neonates failing in binaural screening and monaural screening) was 11. 8%; univariate analysis showed that the differences of birth weight,gestational age,the number of births,external ear malformation,oxygen therapy or not after birth,admitted to NICU or not,parental educational level and family income were statistically significant( P〈0. 05); multivariate analysis showed that the differences of the number of births,external ear malformation,and oxygen therapy or not after birth were statistically significant( P〈0. 05 or P〈0. 01). Conclusion: The number of births,external ear malformation,and oxygen therapy or not after birth are influencing factors of failing in TEOAE screening for infants; twins or multiple births,external ear malformation,and oxygen therapy after birth should be paid more attention to,close follow- up and timely intervention should be conducted to avoid the effect of hearing loss on children's development.
