微造瘘经皮肾镜碎石取石技术对婴幼儿肾结石的安全性及有效性

摘　　要：目的：探讨在婴幼儿肾结石患儿中采用微造瘘经皮肾镜碎石取石技术的疗效及安全性。方法：对2012年4月~2014年9月河北医科大学附属哈励逊国际和平医院收治的行微造瘘经皮肾镜碎石取石术治疗的60例婴幼儿肾结石患儿进行回顾性分析,其中30例术后不留置瘘管治疗（A组）,30例术后留置瘘管进行治疗（B组）,对比两组患儿的临床疗效及安全性。结果：两组患儿一次性结石清除率、穿刺时间、并发症发生率及术后尿素氮、血肌酐等肾功能指标水平比较差异均无统计学意义（P〉0.05）;但A组患儿手术出血量显著少于B组,且手术时间、住院时间等均显著短于B组患儿,差异均有统计学意义（P〈0.05）。结论：在婴幼儿肾结石患儿中采用微造瘘经皮肾镜碎石取石技术的疗效确切,而术后不留置瘘管不仅能缩短手术、住院等时间,降低出血的风险,且不影响手术安全性与有效性。Objective： To explore the curative effect and effectiveness of minimally invasive percutaneous nephrolithotomy lithotripsy technology for renal calculus in infants. Methods： Sixty infants with renal calculus treated by minimally invasive percutaneous nephrolithotomy lithotripsy technology in the hospital from April 2012 to September 2014 were analyzed retrospectively,then the infants were divided into group A（ no fistula treatment after surgery） and group B（ indwelling fistula after surgery）; the clinical efficacies and safeties in the two groups were compared. Results： There was no statistically significant difference in the disposable stone clearance rate,puncture time,incidence rate of complications,postoperative urea nitrogen and blood creatinine levels between the two groups（ P〉0. 05）; the amount of blood loss during operation in group A was significantly less than that in group B,the operation time and hospitalization time in group A were shorter than those in group B,there were statistically significant differences（ P〈0. 05）. Conclusion： The curative effect of minimally invasive percutaneous nephrolithotomy lithotripsy technology for renal calculus in infants is good,no indwelling fistula after surgery can shorten operation time and hospitalization time and reduce the risk of bleeding,and it cannot affect safety and effectiveness of the operation.
