电磁导航定位法在危重患者留置鼻空肠营养管中的应用

摘　　要：目的探讨电磁导航定位法在危重患者床旁留置鼻空肠营养管的有效性和安全性。方法 回顾性分析2012年1月至2013年12月本院ICU在床旁置鼻空肠营养管行营养支持患者90例临床资料,按置入的鼻空肠营养管类型分为2组,对照组50例盲视置入FLOCARE-螺旋型鼻空肠营养管,实验组40例采用电磁导航定位法置入Corflo-10FR鼻空肠营养管。比较两组患者置管成功率、置管成功者所需置管时间、置管后生命体征恢复时间。结果 实验组40例患者中37例置管成功,成功率92.5%,对照组50例患者中38例置管成功,成功率76%,两组差异有统计学意义(x^2=4.356,P=0.037);两组置管成功者所需置管时间差异有统计学意义(t=21.782,P<0.001),置管后生命体征恢复时间差异具有统计学意义(t=-7.754,p<0.001)。结论采用电磁导航定位法为危重患者床旁留置鼻空肠营养管操作时间短、安全、成功率高,值得推广使用。Objective To discuss the efficacy and safety of electromagnetic navigation localization method in in- dwelling jejunum feeding tube through nose for critical patients. Methods Retrospectively analyzed clinical data of 90 critical patients indwelled jejunum feeding tube for nutrition support in ICU of our hospital from January 2012 to December 2013. These participants were divided into two groups according to jejunum feeding tube type,50 in the control group and 40 in the experimental group. The patients in the control group were indwelled FLOCARE- whorl tubes blindly and those in the experimental group were indwelled Corfio-10FR tubes with assistant of the electromagnetic navigation localization method. The rate and required time of successful cases,and recovery time of vital signs were compared between the 2 groups. Results The rate of successful cases（92.5%） of the experimental group was significantly higher（X2=4.356,P=-0.037） than that of the control group（76%）. The experimental group needed significantly shorter required time（t=21.782,P〈0.001） and shorter recovery time of vital signs（t=-7.754,P〈0.001） than the control group. Conclusion It is more efficacy,safer and easier to succeed to indwell jejunum feeding tube through nose with assistant of Electromagnetic navigation localization method for critically ill patients,which should be promoted in clinic.
