摘 要:目的对国内外系统性红斑狼疮患者并发感染的危险因素进行系统分析。方法 检索外文数据库PubMed、EMBASE、OVID,中国学术期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、万方数据库获得已公开发表的国内外系统性红斑狼疮并发感染相关危险因素的病例对照的研究文献,筛选出符合评价标准的文献,采用RevMan 5.2软件进行meta分析,并计算各危险因素的合并比值比(OR)及95%置信区间(95%CI)。结果 共纳入14篇病例对照研究,累计病例组1008例、对照组1457例。系统性红斑狼疮并发感染的各危险因素的合并OR值(95%CI)分别为:年龄2.09(1.50,2.91)、病程1.10(0.77,1.58)、激素用量3.05(1.15,8.07)、免疫抑制剂使用2.01(1.21,3.32)、补体C3的含量0.59(0.42,0.82)、白细胞数0.37(0.28,0.48)、性别0.98(0.61,1.56)、系统性红斑狼疮活动度评分0.44(0.32,0.59)、受累器官数2.53(1.87,3.42)、C反应蛋白0.18(0.10,0.31)、抗生素种类5.82(0.76,44.31)。结论系统性红斑狼疮并发感染的危险因素主要是年龄、免疫抑制剂使用、激素的用量、补体C3含量、白细胞数、系统性红斑狼疮活动度的评分、受累器官数和C反应蛋白,而性别、病程、抗生素种类在本研究中显示无统计学意义,这仍有待于临床开展更大样本更严谨的病例对照研究进一步证实。Objective To systematically analyze the risk factors of infections in patients with systemic lupus ery- thematosurs (SLE). Methods The case-controlled studies related to the risk factors of SLE were retrieved through databases such as PubMed,EMBASE,OVID,China Academic Journal Full-Text Database(CNKI),Chinese Biomedical Literature Database(CBM) and Wanfang database. The pooled odd ratio(OR) values were calculated using Meta-anal- ysis by RevMan 5.2 software. Results Fourteen case-controlled studies were included involving 1008 cases in the case group and 1457 cases in the control group. The pooled OR values and 95% confidence intervals of each risk factor of infections in SLE patients were as follows: age 2.09 (1.50,2.91),illness course 1.10(0.77,1.58),the dosage of hormone 3.05 (1.15,8.07),the use of immune inhibitors 2.01 (1.21,3.32),C3 level 0.59(0.42,0.82),the count of white blood cell (WBC) 0.37 (0.28,0.48),gender 0.98 (0.61,1.56),SLEDAI score 0.44(0.32,0.59),the number of involved organs 2.53(1.87,3.42),the level of C-reactionprotein(CRP) 0.18 (0.10,0.31) and the kinds of antibiotics 5.82 (0.76,44.31). Conclusion The main risk factors of infections in SLE patients are age,the use of immune inhibitors,the dosage of hormone,C3 level,WBC count,SLEDAI score,the number of involved organs and CRP level,while meta-analyses failed to support the factors of illness course,gender and the kinds of antibiotics.
