腕关节MRI成像对早期类风湿性关节炎诊断价值

摘　　要：目的评价腕关节MRI成像对早期类风湿性关节炎(RA)诊断价值。方法收集早期RA病人88例的临床资料,所有病人均行腕关节MRI平扫和增强扫描,观察各种MRI异常征象并分析其对早期RA的诊断价值。结果 MRI平扫异常征象主要表现为:滑膜增生85例,骨髓水肿72例,关节腔积液62例,肌腱炎60例,骨质侵蚀57例;增强扫描显示血管翳强化85例。滑膜增生和血管翳强化诊断早期RA灵敏度高。结论 MRI成像对早期RA的病理改变较敏感,可作为早期RA诊断的首选。Objective To evaluate the value o（ MRI of the wrist in early diagnosis of early-stage rheumatoid arthritis （RA）. Methods Clinical data of 88 patients with early RA were collected. MRI plain scan and enhanced scan were carried out for the wrist joints of the patients, the abnormal signs of MRI were ohserved and their value in the diagnosis of early RA was assessed. Results The abnormal signs of plain MRI were manifested was follows., synovial hyperplasia （85 cases）, bone marrow edema （72 cases）, joint effusion （62 cases）, myotenositis （60 case） and hone erosion （57 cases） ; enhanced scan showed 85 eases with pannus enhancement. The synoviai hyperplasia and pannus enhancement had a high sensitivity in the diagnosis of early-stage RA. Conclusion MRI is relatively sensitive to the pathological changes of rheumatoid arthritis, which can he used as the first choice for the diagnosis of the disease.
