摘 要:目的:了解职业人群细胞角蛋白19片段(CYFRA21_1)、神经元特异性烯醇化酶(NSE)分布情况及其在肺癌筛查的作用,以为开展大规模人群筛检提供理论线索。方法采用 CYFRA21_1、NSE 试剂盒对辖区2014年1-12月年度体检的机关、企事业单位的职业人群血清进行检测。结果 CYFRA21_1(D K-S =0.09,P 〈0.01)、NSE(D K-S =0.09, P 〈0.01)不服从正态分布,中位数分别为1.92 ng/ml 和12.51 ng/ml;年龄分组中,除了51-60岁组的 NSE(13.77±2.88)ng/ml 呈正态分布之外,其他均不服从正态分布;不同性别 CYFRA21_1(Z =-4.10,P 〈0.01)、NSE(Z =-9.03,P 〈0.01)分布差异有统计学意义。NSE、CYFRA21_1检测值异常个体分别占10.12%(234/2311)和9.26%(214/2311),其中 NSE 与 CYFRA21_1检查异常结果相关癌症确诊率分别为0.43%和0.47%。结论虽然 CYFRA21_1、NSE 已经被证实为肺癌诊断的重要生物标志物,由于其人群表达差异性大以及特异性不高,其运用于正常人群和高危人群的肺癌筛查仍值得商榷。Objective To explore the distribution of cytokeratin segment 1 9 (CYFRA21_1)and neuron-specific enolase (NSE )in the occupation crowd and the effect on the lung cancer screen and provide the clue for lung cancer screening in the large scale population. Methods CYFRA21_1 and NSER kits were used to test the serum from employees of the authority units,institutions and enterprises for health check in Guangming District between January and December 2014. Results The distribution was not normal for CYFRA21 _ 1 (D K-S =0.09,P 〈0.01)and NSE(D K-S =0.09,P 〈0.01)with the media of 1.92 ng/ml and 12.5 1 ng/ml, respectively for them.In the age subgroup,the distribution of all the subgroups of the NSE and CYFRA21_1 was not normal except the 5 1-60 age group of NSE (13.77±2.88 ng/ml).And the different distribution were statistics in the gender for the CYFRA21 _ 1 (Z = - 4.10,P 〈 0.01 )and NSE (Z = - 9.03,P 〈 0.01 ). 10.12% (234/231 1)and 9.26 %(214/231 1)of all participants had abnormal results for NSE and CYFRA21_1 and the proportion were 0.43% and 0.47% of them with the diagnosis of the relative cancer. Conclusion Although CYFRA21_1 and NSE are certified to be important biological markers in the diagnosis of lung cancer, it is also questionable for applying them for lung cancer screening in normal and high risk population because of their low specificity and large different expression in the population.
