重庆三峡中心医院2008-2014年儿童乙型肝炎病毒感染情况分析

摘　　要：目的:初步了解重庆万州地区2008-2014年儿童乙型肝炎病毒(HBV)感染的变化趋势和免疫现状,为HBV感染的防控提供理论依据。方法:收集2008-2014年重庆三峡中心医院儿科收治的42 973例儿童(<14岁)的HBV血清学标志物(HBs Ag、HBs Ab、HBe Ag、HBe Ab、HBc Ab)检测报告(ELISA法),采用回顾性研究方法进行分析。结果:总体HBV感染检出率即HBs Ag阳性率为5.65%(2 428/42 973),男女分别为5.80%和5.43%(P>0.05)。HBs Ab阳性率53.69%(23 071/42 973),大三阳(HBs Ag、HBe Ag、HBc Ab均阳性)比例0.99%(425/42 973),小三阳(HBs Ag、HBe Ab、Hbc Ab均阳性)比例0.55%(237/42973),HBV血清学标志物全阴性比例15.55%(6 681/42 973)。2008-2014年各年份HBs Ag阳性率依次为4.56%、4.39%、1.33%、4.92%、5.45%、9.70%、5.61%。结论:2008-2014年重庆三峡中心医院儿童HBV感染率(HBs Ag阳性率)低于2006年我国普通人群HBV感染率,但有上升趋势,HBs Ab阳性率高于2006年我国普通人群HBs Ab阳性率,提示重庆万州地区的HBV感染防控工作需继续加强。Objective： To investigate the condition of hepatitis B virus （HBV） infection and the immune station of children under 14 years old in Wanzhou region of Chongqing from 2008 to 2014, and provide theoretical foundation for the prevention and control of HBV. Methods： ELISA method was used to detect the hepatitis B virus serology markers （HBsAg, HBsAb, HBeAg, HBeAh, HBcAh ） in 42,973 children selected from Chongqing Three Gorges Central Hospital. Results： The average positive rate of HBsAg was 5.65% between 2008 and 2014, the average positive rate of HBsAg in boys and gifts were 5.80% and 5.43% respectively. Positive rate of HBsAb was 53.69%;6,681 cases were negative in HBVM （15.55%）; 425 cases were positive in HBsAg, HBeAg and HBcAb （0.99%）; 237 cases were positive in HBsAg, HBeAb and HBeAb （0. 55% ）. The positive rate of HBsAg from 2008 to 2014 were 4. 56%, 4. 39%, 1.33%, 4. 92%, 5.45%, 9. 70%, 5.61%. Conclusion： Among the 42,973 children, the average infection rate of HBV was lower than that of common crowds in our country in 2006. The positive rate of HBsAh was higher than that of common crowds in our country in 2006. It is suggesting that the HBV prevention and control work need to be strengthened.
