痰热清注射液佐治小儿肺炎支原体肺炎对辅助T细胞及其细胞因子的影响

摘　　要：目的：观察痰热清注射液佐治小儿肺炎支原体肺炎（MPP）对辅助T细胞Th1、Th2及其细胞因子IFN-γ、IL-4的影响,评价其疗效。方法：选取2013年1月至2014年6月我院收治的MPP患儿180例,按入院顺序单双号随机分为对照组和观察组各90例。两组患儿均给予对症治疗和阿奇霉素10 mg/（kg·d）口服,观察组加用痰热清注射液0.3～0.5 m L/（kg·d）静脉滴注,连续治疗7 d。观察两组患儿发热、咳嗽、肺部啰音、X线胸片阴影消失时间,并于治疗前和治疗后第3天、第7天测定患儿血清及痰液中Th1、Th2、IFN-γ、IL-4的含量,评价疗效。结果：观察组治愈率和总有效率均高于对照组（P均〈0.05）;发热、咳嗽、肺部啰音、X线胸片阴影消失时间均短于对照组（P均〈0.05）;治疗后第3天、第7天血清Th1、IFN-γ水平及Th1/Th2、IFN-γ/IL-4值,痰液IFN-γ水平及IFN-γ/IL-4值均低于对照组（P均〈0.05）。结论：痰热清注射液能调节Th1/Th2平衡,促进MPP患儿康复。Objective ： To observe the influence of Tanreqing injection for the helper T cells （ Thl, Th2 ） and their cytokines of children with Mycoplasma pneumonia （MPP）, and estimate the curative effect. Methods： One hundred and eighty cases of children with MPP were randomly divided into control group and observation group according to the order of admission, each group had 90 cases. Children in control group were treated with azithromycin, and those in observation group were treated with azithromycin combined with Tanreqing injection. Then the disappearance time of fever, cough, pulmonary rales, X-ray shadow of two groups were counted, and the serum and sputum levels of Thl, Th2 and IFN-γ, IL-4 were detected and compared before and after treatment on the 3th day, 7th day. Results： The cure rate and effective rate of observation group were higher than control group, and the disappearance time of fever, cough, pulmonary rales, X-ray shadow were shorter than control group （P〈0.05）. After 3 days, 7 days of treatment, the levels of Th1, IFN-γ, and Thl/Th2, IFN-γ/IL-4 in serum of observation group were lower than those in control group, and the levels of IFN-γ and IFN-γ/IL-4 in sputum were lower than those in control group （P〈0.05）. Conclusion： Tartreqing injection can regulate the balance of Th1/Th2, promote the rehabilitation of children with MPP.
