无痛手法松解联合穴位和肩关节腔注射治疗重症肩关节周围炎40例疗效观察

摘　　要：目的观察无痛手法松解联合穴位和肩关节腔注射治疗重症肩关节周围炎的临床疗效。方法将80例重症肩关节周围炎患者随机分为2组。对照组40例采用肩髃、肩贞、巨骨穴、阿是穴和肩关节腔注射治疗,治疗组40例在肌间沟臂丛神经阻滞下行无痛手法松解,而后行上述穴位和肩关节腔注射治疗。2组均治疗2个月后观察疗效、治疗前后肩部疼痛视觉模拟评分(VAS)和肩关节活动度评分(Mallet)。结果治疗组疗效优于对照组(P<0.05)。治疗组治疗后VAS评分及Mallet评分改善优于对照组治疗后(P<0.05)。结论臂丛神经阻滞下无痛手法松解联合穴位和肩关节腔注射治疗重症肩关节周围炎疗效确切。Objective To observe the clinical effects of painless manipulation release combined with point and intra -articular injection on severe shoulder periarthritis .Methods 80 patients with severe shoulder periarthritis were randomly divided into two groups .40 patients in control group received point （Jianyu,Jianzhen,Jugu,Ashi） and intra -articular injection.40 patients in treatment group received painless manipulation release undergoing brachial plexus block combined with the same injection as control group .The course was one month in two groups .The clinical effects,visual analog pain score （VAS） and shoulder joint activity score （Mallet score） were measured after treatment in two groups.Results The effect in treatment group was superior to that in control group （P 〈0.05）.The improve-ment of VAS score and Mallet score after treatment in treatment group was superior to that in control group （ P 〈0.05）.Conclusion Painless manipulation release combined with point and intra -articular injection on severe shoulder periarthritis has an exact effect .
