通心络胶囊治疗老年冠心病经皮冠状动脉介入治疗术后心绞痛临床观察

摘　　要：目的观察通心络胶囊治疗老年冠心病经皮冠状动脉介入治疗(PCI)术后心绞痛的临床疗效。方法将老年冠心病PCI术后心绞痛患者116例随机分为2组,对照组56例予阿司匹林、氯吡格雷、单硝酸异山梨酯、酒石酸美托洛尔、阿托伐他汀钙等常规治疗,治疗组60例在对照组基础上加用通心络胶囊。2组治疗时间均为6个月,比较治疗前后心绞痛症状疗效、心电图疗效,测定超敏-C反应蛋白(hs-CRP)、一氧化氮(NO)、内皮素-1(ET-1)及血脂指标水平变化。结果治疗组治疗后心绞痛症状疗效、心电图疗效优于对照组(P<0.05);治疗后2组血浆NO水平升高(P<0.05),ET-1、hs-CRP水平降低(P<0.05),且2组组间比较差异有统计学意义(P<0.05)。2组治疗后总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平均较本组治疗前下降(P<0.05),且治疗组TC、LDL-C较对照组下降明显(P<0.05)。结论通心络胶囊可显著减少老年冠心病PCI术后心绞痛发作频率,同时改善患者心电图ST-T变化,调整血脂,升高血浆NO水平,降低ET-1、hs-CRP水平,是辅助治疗冠心病PCI术后心绞痛的一种安全有效的方法。Objective To observe the clinical effects of Tongxinluo capsule on angina in aged patients with coronary heart disease （CHD） after percutaneous coronary intervention （PCI）.Methods 116 aged patients with coronary heart disease after percutaneous coronary intervention were randomly divided into two groups .56 pa-tients in control group were treated by routine therapy such as aspirin ,clopidogrel,isosorbide mononitrate,metoprolol tartrate and atorvastatin calcium tablets .Patients in treatment group were treated by Tongxinluo capsule on the basis of control group treatment.The course was six months in two groups .The changes of symptoms of angina ,ECG effica-cy,the levels of high -sensitive c -reactive protein （hs -CRP）,nitric oxide （NO）,endothelin -1 （EP -1） and blood lipid indices were compared between before and after treatment in two groups .Results The curative effects of angina and ECG after treatment in treatment group were superior to those in control group （P 〈0.05）.The level of serum NO after treatment was increased （P 〈0.05）,the levels of ET -1 and hs -CRP were decreased （P 〈0.05）, and there were significant differences between two groups （P 〈0.05）.The levels of total cholesterol （TC）,triglycer-ide （TG） and low density lipoprotein （LDL -C） after treatment were decreased （P 〈0.05）,and the decreased of TC,LDL -C in treatment group was more obvious than that in control group （P 〈0.05）.Conclusion Tongxinluo capsule can significantly reduce the occurrence frequency of angina in aged patients with coronary heart disease after percutaneous coronary intervention ,and improve st -t changes of ECG,regulate blood lipids,increase the serum NO level and decrease the level of ET -1 and hs -CRP,is a safe and effective adjuvant therapy .
