半髋关节置换患者的失血和输血状况研究

摘　　要：目的:分析半髋关节置换患者失血和输血的状况,探讨能降低术后患者贫血的有效措施。方法:对2010年1月至2014年1月行半髋关节置换的80例胫骨颈骨折患者的临床资料进行回顾性分析,分析患者术前和术后的血常规、术中和术后的失血及输血量。结果:80例患者术后1d的平均失血量占患者术后3d平均总失血量的71.64%;合并高血压患者的总失血量及隐性失血量均明显高于不伴高血压的患者;术前血红蛋白(Hb)<110g/L的患者输血的比例比较高。结论:半髋关节置换患者的隐性失血的比例很高,合并高血压的患者的总失血量及隐性失血量均明显增加,对Hb较低的患者要提前做好输血的准备。Objective： To analyze the status of blood loss and transfusion in hemiarthroplasty for the patients with femoral neck fracture.To explore the effective measures to reduce anemia in postoperative patients.Method： The clinical data of the patients with femoral neck fracture who had done hemiarthroplasty from Jan. 2010 to Jan. 2014 were retrospectively analyzed. Blood loss and blood transfusion of intraoperative and postoperative were recorded. Result： The average total blood loss of the patients postoperative after one day was 71.64% of the average total blood loss of postoperative 3 days. The total blood loss and the invisible blood loss of the patients with hypertension were significantly higher than the patients without hypertension.The proportin of transfusion of the patinents of Hb level less than 110 g / L was relatively high. Conclusion：The proportin of hidden blood loss of the patients with hemiarthroplasty is high. The total blood loss and the hidden blood loss of the patients with high blood pressure are significantly increased.The patients with lower Hb is to prepare for transfusion in advance.
