糖皮质激素联合常规疗法治疗结核性胸膜炎56例临床观察

摘　　要：目的观察糖皮质激素联合常规疗法治疗结核性胸膜炎的疗效。方法选择2011年4月至2014年4月收治的患者112例,随机分为观察组和对照组,各56例。对照组给予常规疗法,观察组加用糖皮质激素。结果观察组的全身中毒症状缓解时间、胸腔积液吸收时间、住院时间均明显短于对照组(P<0.05);观察组治疗6个月末胸膜粘连增厚发生率明显低于对照组(P<0.05);观察组的总有效率为92.96%,明显高于对照组的82.14%(P<0.05)。结论糖皮质激素联合常规疗法治疗结核性胸膜炎,可明显缩短患者全身中毒症状缓解时间、胸腔积液吸收时间及住院时间,降低患者的胸膜粘连增厚发生率,提高治疗总有效率,值得推广。Objective To observe the clinical efficacy of glucocorticoid combined with the routine therapy in the treatment of tuberculous pleurisy. Methods Totally 112 patients with tuberculous pleurisy in our hospital from April 2011 to April 2014 were selected and randomly divided into the observation group and the control group. The control group was treated with the routine therapy while the observation group was treated with glucocorticoid combined with the routine therapy. Results The alleviation time of poisoning symptoms, pleural effusion absorption time and hospitalization time in the observation group were significantly shorter than those in the control group（ P 〈 0. 05）;the incidence rate of pleural adhesion and thickening at the end of 6- month treatment in the observation group was significantly lower than that in the control group（P 〈 0. 05）; the total effective rate in the observation group was 92.96%, which was significantly higher than 82. 14% in the control group（P 〈 0. 05）. Conclusion Glucocorticoid combined with the routine therapy in the treatment of tuberculous pleurisy can significantly shorten the alleviation time of systemic poisoning symptoms, pleural effusion absorption time and hospitalization time, reduces the incidence rate of pleural adhesion and thickening, and enhances the total effective rate and is worth popularization in clinic.
