自拟芪术汤治疗肥胖相关性高血压病临床疗效观察

摘　　要：目的:观察自拟芪术汤治疗肥胖相关性高血压病的临床疗效。方法:80例符合中西医纳入标准的肥胖相关性高血压病患者随机分为两组,对照组给予常规一线抗高血压药代文(根据患者血压情况确定药量)治疗,试验组在常规西药代文基础上加服自拟芪术汤100 m L/次,2次/d,早晚餐后半小时服用。疗程为2个月。观察两组治疗前后临床疗效、中医症状疗效积分以及安全性指标。结果:治疗2个月后,试验组(自拟芪术汤组)对血压改善显效率达52.5%,总有效率达87.5%,对收缩压(SBP)和舒张压(DBP)较对照组有显著降低(P<0.01);对肥胖测量指标,试验组较对照组对体重指数、腰围、腰臀比有显著性改善(P<0.01);在改善血脂方面对TG、CHOL、LDL-C及LP(α)均降低,HDL-C显著升高,优于对照组(P<0.05,P<0.01)。自拟芪术汤组合常规降压药治疗40例"脾失健运"型肥胖相关性高血压病患者,中医症状显效率32.5%,总有效率97.5%。临床用药安全,未发现不良反应。结论:自拟芪术汤合常规降压药治疗肥胖相关性高血压病降压效果显著,且可减轻体重,改善脂代谢紊乱,疗效安全。Objective：To use the Yunpi Shengqing treatment of obesity-associated hyper-tension,and to explore the effectiveness of the treatment and the changes of relevant indicators. Methods：Clinical observation：Patients who are the clinical diagnosis of obesity complicated with hypertension and the symptoms were spleen failing in transportation were randomly divided into experimental and control groups,the experimental group with 40 cases have antihypertensive drug valsartan capsules together Zini Qizhu Tang,while the control group of 40 patients only have antihypertensive medicine valsartan capsules,all the patients are taken medication for four weeks for a course,two courses of treatment. Results：The effective rate of experimental group（Yunpi Shengqing group）on blood pressure improved markedly was 52.5%,total effective rate was 87.5%. Systolic blood pressure（SBP）and the diastolic blood pressure（DBP）significantly reduced than the control group（P0.01）;after two courses of treatment,the experimental group had significant difference than the control group（P0.01）on the measurement indicators（BMI,WC,WHR）;in improving the lipid profile,TG and CHOL,LDL-C,HDL-C,Lp（α）had significantly better than the control group（P0.05,P0.01）. Yunpi Shengqing recipe together common antihypertensive drugs to treat 40 cases of spleen failing in transportation patients with obesity-associated hypertension,effective rate of clinical symptoms was 32.5%,the total effective rate was 97.5%. Clinical safety and found no adverse reactions.（2）Experimental study on the mechanism：Compared with DIO group,Zini Qizhu Tang group＇s body mass index,fat coefficient and IRI decreased（P0.05,P0.01）,in the serum and the homogenized fat leptin and adiponectin increased（P0.01）,in the serum and the homogenized fat TNF-α decreased（P0.05,P0.01）,in the homogenized fat SOCS-3 decreased（P0.01）;in the serum and the homogenized fat,dampness expectorant group＇s TNF-α decreased（P0.05）,in the serum adiponectin
