弹性超声蓝色面积百分比在乙肝纤维化分级中的应用研究

摘　　要：目的：分析实时超声弹性图像的蓝色面积百分比在乙型肝炎后肝纤维化分级中的诊断价值。方法选取2013年4月～2014年10月期间在重庆三峡中心医院及四川省达州市中心医院就诊且有病理分期的乙型肝炎患者128例,对乙型肝炎患者的肝脏进行实时超声弹性成像,然后按蓝色面积百分比进行分级,与病理分期进行相关性分析。结果按照实时超声弹性图像蓝色面积百分比对乙型肝炎纤维化进行分级与病理分期高度相关（r=0.89,P=0.000）。实时超声弹性成像分级诊断与病理诊断对比结果显示：特异性为73.33%（22/30）,准确性为86.72%（111/128）,敏感性为90.82%（89/98）。结论按照实时超声弹性图像蓝色面积百分比对乙型炎后肝纤维化进行分级与病理分期具有很高的相关性,对乙型肝炎后肝纤维化的诊断有很高的敏感性、准确性及特异性,可以判断乙型肝炎后有无肝纤维化及肝硬化。Objective To investigate the diagnostic value of the area percentage share of the blue region in ultrasonic real-time tissue elastography （RET） in hepatitis B virus-related liver fibrosis grading. Methods From April 2013 to Oc-tober 2014 in Chongqing Three Gorge Central Hospital and Dazhou Central Hospital of Sichuan Province during the period of treatment 128 hepatitis B patients with pathologic examination results were scanned with RET, and classified based on the area percentage share of the blue region in RET. Then the correlation between RET grading and patholog-ical staging was analyzed. Results There was high correlation between RET grading based on the area percentage share of the blue region and pathological staging for evaluation of liver fibrosis due to hepatitis B （r = 0.89, P= 0.000）. In the comparative analysis between RET grading and pathological staging, the specificity was 73.33% （22/30）, the accu-racy was 86.72%（111/128）, the sensitivity was 90.82% （89/98）. Conclusion With a high degree of sensitivity, accuracy and specificity, there is a high consistency between RET grading based on the area percentage share of the blue region and pathological staging for hepatitis B virus-related liver fibrosis, which is available for the judgment of liver fibrosis or cirrhosis after hepatitis B.
