布地奈德福莫特罗粉吸入剂联用孟鲁司特治疗Ⅲ、Ⅳ级慢性阻塞性肺疾病临床观察

摘　　要：目的观察布地奈德福莫特罗粉吸入剂(信必可)联用孟鲁司特治疗Ⅲ、Ⅳ级慢性阻塞性肺疾病(COPD)患者的疗效及安全性。方法将70名确诊为稳定期的Ⅲ、Ⅳ级COPD患者随机分为治疗组和对照组各35例,治疗组使用信必可联用孟鲁司特治疗,对照组使用信必可治疗。在治疗前、治疗后1个月、3个月行血气分析、肺功能检测、呼吸困难严重程度评分变化、自我评估测试评分变化。结果治疗1个月及3个月后,2组的肺功能指标皆明显改善;且治疗组优于对照组,2组治疗后Pa O2均高于治疗前,Pa CO2低于治疗前,且治疗组变化幅度大于对照组,治疗后,2组m MRC评分及CAT均明显低于治疗前,且治疗组低于对照组,出现加重症状的比例为8.6%低于对照组的17.1%,差异均有统计学意义(P<0.01)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论Ⅲ、Ⅳ级COPD患者使用信必可联用孟鲁司特综合治疗效果优于信必可,值得临床推广应用。Objective To study the efficacy and safety of budesonide and formoterol powder for inhalation （ symbicort turbuhaler ） combined with montelukast in treatment of Ⅲ,Ⅳ level of chronic obstructive pulmonary disease .Methods 70 pa-tients with Ⅲ,Ⅳ grade COPD were randomly divided into treatment group and control group ,each of 35 cases.Treatment group was given with symbicort turbuhaler combined with montelukast;Control group was given with symbicort turbuhaler .The blood gas analysis,lung function,dyspnea severity scoreand self-assessment test score were respectively detected at before treatment , 1 month,3 months after administration .Results The PaO2 ,PaCO2 ,pulmonary function ,mMRC score and CAT were improved in two groups compare with those before treatment ,especially in treatment group;The aggravate symptoms appear proportion of treatment group was 8.6%lower than 17.1%of control group,the differences were statistically significant （P〈0.01）.The in-cidence of adverse reactions of two groups was no significant difference （P〉0.05）.Conclusion The effects of combination of symbicort turbuhaler combined with montelukast in treatment of stable Ⅲ,Ⅳ grade chronic obstructive pulmonary disease is bet-ter than symbicort turbuhaler ,and is worthy of popularization and application .
