摘 要:目的 探讨肿瘤坏死因子(Tumor Necrosis Factor-α,TNF-α)G-308A和T-1031C单核苷酸多态性与广东地区汉族人群冠心病遗传易感性的关系。方法 应用SNPscan TM多重SNP分型试剂盒对广东地区汉族人群783例冠心病患者和749例正常对照样本DNA的2个SNP(G-308A和T-1031C)进行分型。应用卡方检验和Logistic回归模型进行统计分析。结果 TNF-αG-308A和T-1031C等位基因和基因型的频数分布在冠心病组和对照组间差异无统计学意义(P〉0.05);多因素Logistic回归模型检验发现,校正混淆因素(年龄、性别、高血压、糖尿病、吸烟史)后,TNF-αT-1031C位点基因型在隐性模型下对冠心病有显著影响(P=0.045,OR=2.10,95%CI:1.02~4.33),TNF-αG-308A和T-1031C位点的-308G/-1031T单体型对冠心病有显著影响(P=0.029)。但经max(T)permutation多重检验校正后,TNF-α基因型和单体型结果均不再显著(校正后P值分别为0.073和0.069)。结论 本研究尚未证实TNF-αG-308A和T-1031C位点多态性与广东地区汉族人群冠心病有关联性。Objective To study the association between tumor necrosis factor-α(TNF-α) G-308 A and T-1031 C single nucleotide polymorphisms and Coronary Artery Disease(CAD) in Guangdong Han population. Methods Two SNPs(G-308 A and T-1031C) in 783 CAD patients and 749 healthy individuals(as controls) recruited from Guangdong Han population were analyzed by SNPscanTMmultiple SNP genotyping assays, and the statistical analysis was conducted by using Chi-square test and logistic regression model. Results There were no significant difference between patients with CAD and health controls in terms of allelic and genotypic distribution at TNF-α G-308 A and T-1031 C polymorphic loci(all P〈0.05). After adjusting several confounding factors(age, gender, hypertension, diabetes and smoking history), multivariate logistic regression model demonstrated that TNF-αT-1031 C was associated with CAD under the recessive model(P=0.045, OR=2.10, 95% CI: 1.02-4.33), and haplotype-308G/-1031 T at the two loci were associated with CAD. However, these findings became negative after correcting for multiple testing by max(T) permutation(corrected P=0.073, 0.069 for genotype and haplotype respectively). Conclusion This study lacks adequate evidence to support the association of the polymorphisms at TNF-α G-308 A and T-1031 C with CAD in Guangdong Han population.
