评价眩晕宁加减结合氟桂利嗪对眩晕症的完全随机对照试验研究

摘　　要：目的 观察并比较天麻眩晕宁颗粒加减结合盐酸氟桂利嗪治疗高血压性眩晕的临床效果。方法 将70例高血压性眩晕患者随机分为实验组和对照组各35例,实验组口服天麻眩晕宁颗粒（每次8 g,每日3次）联合盐酸氟桂利嗪胶囊（每次10 mg,每晚1次）。对照组仅服用眩晕宁颗粒,用法同实验组。2个月后观察两组患者的疗效,比较两组治疗前后TCD变化、血液流变学改变。结果 实验组显效27例,有效6例,无效2例,总有效率为94.29%;对照组显效20例,有效7例,无效8例,总有效率为77.14%;两组总有效率比较差异有统计学意义（P〈0.05）。经治疗后TCD检查结果提示两组患者的左右椎动脉（LVA、RVA）及基底动脉（BA）的平均流速均有明显提高（t=5.76~6.43,P〈0.05）;实验组相较于对照组有显著差异（tLVA=2.67,P〈0.05;tRVA=3.89,P〈0.05;tBA=2.87,P〈0.05）。两组患者治疗后血液流变学指标（血液粘度,红细胞压积、红细胞聚集指数及纤维蛋白原）均有明显降低（t=2.06~8.24,P〈0.05）;实验组相较于对照组有显著差异（t值分别为2.00、3.04、2.23、3.07,P〈0.05）。结论 天麻眩晕宁颗粒联合盐酸氟桂利嗪能显著提高高血压性眩晕症的疗效,提高椎-基底动脉供血,降低血液粘滞度,无明显副作用,值得临床推广。Objective To observe and compare the clinical effect of Tianma Xuanyunning particles with or without flunarizine in treating hypertension vertigo. Methods The 70 cases of hypertension vertigo were randomly divided into an experimental group and a control group, both with 35 cases. The experimental group was treated with Tianma Xuanyunning particles（8g, tid） combined with flunarizine Hydrochloride capsules（10mg, qn）. The Control group was asked to take Xuanyunning particles（8g, tid） only. After 2month, we observed the clinical effect of two groups. The TCD changed as well as the changes in blood rheology were observed before and after the treatment. Results The experimental group was markedly effective in 27 cases, effective in 6 cases and ineffective in 2 cases, the total effective rate was 94.29%; The control group was markedly effective in 20 cases, effective in 7 cases and ineffective in 8 cases, the total effective rate was 77.14%; the difference between the two total effective rate was of statistically significance（P〈0.05）. After the treatment, the results of TCD examination showed that the average flow rate of the left vertebral artery（LVA）, the right vertebral artery（RVA） and the basilar artery（BA） of all patients were significantly improved（t=5.76-6.43,P〈0.05）; There were significant differences between the experimental and the control groups（tLVA=2.67, P〈0.05; tRVA=3.89, P〈0.05;tBA=2.87, P〈0.05）. After the treatment, hemorheology（blood viscosity, hematocrit, erythrocyte aggregation index and fibrinogen）became lower（t=2.06-8.24, P〈0.05） in both groups; There were significant differences between the experimental and the control groups（t= 2.00, 3.04, 2.23, 3.07 respectively, P〈0.05）. Conclusion Tianma Xuanyunning particles combined with flunarizine hydrochloride can significantly improve the therapeutic effect of hypertension vertigo.
