老年机械通气患者脉氧饱和度波形与低血压的相关性

摘　　要：目的观察老年机械通气患者脉氧饱和度波形降中峡下降与低血压的相关性。方法便利抽样法选择2012年4月至2014年3月解放军总医院老年呼吸科病房及ICU收治持续使用机械通气患者36例,依据脉氧饱和度波形降中峡下降程度,将其分为两组,降中峡下降至基线水平(+)设为A组(22例),降中峡下降明显低于基线水平(H)设为B组(14例),记录并比较两组患者的机械通气前的平均动脉压(mean arterial pressure,MAP)、机械通气后降中峡下降时的即刻MAP。,结果机械通气前,两组患者的MAP差异无统计学意义(P>0.05);机械通气后降中峡下降时,两组患者的即刻MAP均低于机械通气前(均P<0.05),B组患者的即刻MAP低于A组(P<0.05)。结论老年机械通气患者脉氧饱和度波形降中峡的下降能够提示低血压的发生。Objective To observe the correlation between pulse oxygen saturation waveform and hypotension in the elderly patients underwent mechanical ventilation（MV）.Methods By convenience sampling,36 elderly patients were selected and divided into two groups based on the decreased level of dicrotic notch. The patients with the dicrotic notch decreased to the baseline （＋） was selected as Group A （n=22）,and the dierotic notch decreased to the baseline （＋＋）was selected as Group B（n= 14）.The mean arterial pres-sure （MAP） before MV,instant MAP of the decrease of dicrotic notch after MV were recorded and compared.Results There was no statistic significance of the MAP between two groups hefore MV（P〉0.05）. The instant MAP of the decrease of dicrotic notch after MV were lower than before MV in both groups （all P〈0.05）.The instant MAP in Group B was lower than Group A （P〈0.05）.Conclusion The decrease of dicrotic notch of pulse oxygen saturation waveform of elerly patients underwent MV can predict the incidence of hypotension.
