摘  要:目的探讨超声剪切波技术在慢性肾脏疾病的诊断以及分期上的应用价值。方法选择2013年1月至2014年6月来我院接受治疗的60例慢性肾脏疾病患者作为CKD组,按病情程度分为5组(CKD1期、CKD2期、CKD3期、CKD4期、CKD5期),同期来院体检肾功能正常的30例健康人员作为正常组。采用超声剪切波对所有研究对象的左肾的VTQ值进行检查,比较分析研究对象的超声检查资料。结果60例CKD患者的血清肌酐、血尿素氮的增高率随着病程的增加逐渐增高,CKD患者的血流量、皮质回声随着病程的增加正常率依次降低,肾脏减小率逐渐增加;与正常组的VTQ值相比较,CKD1—5期各期的VTQ值均小于正常组,且各期VTQ值依次降低,两组比较差异均有统计学意义(P〈0.05)。结论慢性肾脏疾病患者的超声VTQ值均小于正常标准,超声剪切波技术在慢性肾脏疾病的诊断和分期上有很好的应用价值。Objective To investigate the value of shear wave technology of ultrasound in the diagnosis and staging of chronic kidney disease. Methods 60 patients with chronic kidney disease treated at our hospital from January, 2013 to June, 2014 were selected as a CKD group. The CKD patients were divided into five groups according to the severity (CKD1 period, CKD2 period, CKD3 period, CKD4 period, and CKD5 period). 30 healthy persons with normal renal function taking physical examination at our hospital during the same period were selected as a normal group. Ultrasonic shear wave VTQ value of the left kidney of all subjects was checked. The ultrasound data were compared between each two groups. Results The serum level of creatinine and blood urea nitrogen increased with the disease course in the 60 patients with CKD. The normal rates of blood flow and cortical echo in the patients with CKD decreased with the increase of the disease course. The VTQ value was lower in the CKD group than in the normal group; the VTQ values of CKD1-5 were decreasing; with statistical differences between each two groups (P〈0.05). Conclusions Ultrasound VTQ value in patients with chronic kidney disease is lower than normal standard. Ultrasonic shear wave technology has good application value in the diagnosis and staging of chronic kidney disease.
