全厚皮移植整形外科技术在小面积深度烧伤创面修复中的价值

摘　　要：目的探讨全厚皮移植等整形外科技术在小面积深度烧伤创面修复中的近远期疗效,评价其在小面积深度烧伤创面修复中的价值。方法小面积深度烧伤患者26例(30处创面),采用全厚皮移植、各种皮瓣和直接缝合等整形外科技术进行创面修复,观察愈合情况。患者出院后3个月、6个月、12个月随访,调查患者满意度。结果本组26例患者30处创面经整形外科技术修复,其中创面Ι期愈合28处,占93.33%;出现腹部带蒂皮瓣蒂部感染1处,Ⅱ期断蒂手术是经处理后愈合良好;全厚皮移植1处虽然为Ι期愈合,但出现局部花斑改变;创面术后外形良好,肤色与正常皮肤相似,弹性好,随访过程中无明显瘢痕生长情况,关节部位无明显的功能障碍;随访3个月、6个月、12个月,满意度无明显差异(P>0.05)。结论全厚皮移植等整形外科技术在小面积深度烧伤创面修复中可以缩短创面愈合时间,远期满意度高,美学效果理想,在临床上具有广阔的前景。Objective To plastic surgery technology such as observation of full thickness skin grafting Jin Yuan Qi curative effect in the small area deep burn wound repair, evaluate its value in small area deep burn wound repair. Methods Choice the February 2009 to February 2014 in our hospital during the period of 26 patients with small area deep burn wounds（30） as the research object, adopts full thickness skin grafting, all kinds of skin flap and direct suture wound repair, on the plastic surgery techniques such as the observation on the healing of patients, patients discharged from hospital after 3 months, 6 months, 12 months follow-up, patient satisfaction survey. Results In this study, 30 wounds repaired by plastic surgery technique, one wound healing, 28（93.33%）; 1 in the abdomen pedicle flap pedicle of infection,speed cutting phase ii operation is healed well after processing; 1 although full thickness skin graft for a period of healing,but a local tinea change; This group of patients with burn wounds postoperative appearance is good, is similar to normal skin complexion and good elasticity, no obvious scar growth conditions in the process of follow-up, joints have no obvious dysfunction; Followed up for 3 months, 6 months, 12 months satisfaction difference has no obvious statistical significance（P〉0.05）. Conclusion Plastic surgery technology such as full thickness skin graft in the small area deep burn wound repair can shorten wound healing time, long-term high satisfaction, aesthetic ideal, has a broad prospect in clinic.
