糖尿病合并非特异性肾周围脓肿MSCT特征及诊断价值

摘　　要：目的 探讨糖尿病并非特异性肾周围脓肿的MSCT（多层螺旋CT）表现及临床价值.方法 回顾性分析11例经临床治疗证实的非特异性肾周围脓肿患者的CT表现.11例患者首次CT检查均行CT平扫及增强扫描,治疗后行CT平扫复查.观察病变部位、大小、形态、边界及强化特点,分析糖尿病患者与其它病因患者特点.结果 （1）11例肾周围脓肿中,右侧6例,左侧5例.病变包绕患侧肾脏,与患侧肾完全分界不清者4例,部分分界不清者7例,累及患侧腰肌、腹后壁3例.增强后病灶环状强化,其中大小均匀的多发小脓腔2例,以大单房脓腔改变为主边缘见小的脓腔5例,以多发不规则大的多房状脓腔改变4例.（2）6例糖尿病并肾周围脓肿患者,以多发不规则大的多房状脓腔改变4例,大单房脓腔改变为主边缘见小的脓腔2例.结论 糖尿病并肾周围脓肿MSCT表现较其它病因肾周围脓肿,病变范围大,周围组织受累重,内部分隔少.Objective To evaluate the diagnostic value and characteristics of multi-slice CT （MSCT） in diagnosis of non-specific perinephric abscesses in diabetic patients.Methods MSCT features of 11 patients with non-specific perinephric abscesses were analyzed retrospectively.All the patients were performed plain CT scan and enhanced CT scan in the first CT scan,and were performed plain CT scan after treatment.Analyzed the characteristics of patients with diabetes and other causes.Results （1）Of all the 11 patients with perinephric abscesses,there were 6 cases on the right side and 5 cases on the left side.Ipsilateral kidney lesions surrounded entirely unclear boundaries in 4 cases,there were 7 cases of some unclear boundaries,the ipsilateral psoas muscle,abdominal wall were involved in 3 cases.Enhanced lesions ring enhancement,multiple small uniform abscess in 2 cases,large single-room abscess surrounded by multiple small abscess in 5 cases,and multiple irregular large multi-room-abscess in 4 cases.（2）In 6 patients with diabetes,there were multiple irregular large multi-room-abscess in 4 cases,and large singleroom abscess surrounded by multiple small abscess in 2 cases.Conclusion Compared with other causes of renal abscess,MSCT performance of diabetes and renal abscess has l.arger extent of disease,worse surrounding tissue involvement,less internal divider.
