摘 要:目的探讨丹参酮ⅡA对脓毒症大鼠肺损伤的保护作用及其机制。方法选取健康成年Wistar大鼠24只,随机分为假休克对照组(Sham组)、模型组(CLP组)、丹参酮ⅡA组(TanⅡA组)3组各8只。脓毒症组和丹参酮ⅡA组采用盲肠结扎穿孔手术(CLP)法制作大鼠脓毒症模型,术后即刻分别腹腔注射20 mg/d的丹参酮ⅡA磺酸钠注射液和生理盐水,对照组仅做盲肠探查术。实验结束时,收集各组肺组织测定肺组织湿/干质量比(W/D),肺组织匀浆中超氧化物歧化酶(SOD)、谷胱甘肽(GSH)、活性氧自由基(ROS)活性和丙二醛(MDA)含量,光镜下观察各组肺组织形态学改变,采用Western blot检测各组肺组织γ-GCS蛋白的表达。结果光镜下可见Sham组肺组织结构基本上正常,肺泡结构清晰;CLP组肺组织充血水肿,大量炎性细胞浸润,肺泡间隔明显增宽,可见部分肺泡塌陷、不张;TanⅡA组肺组织间质稍增厚,少量炎性细胞浸润,肺组织的炎性病理改变较CLP组显著减轻。CLP组、TanⅡA组的肺组织W/D均较Sham组增高,但TanⅡA组的肺组织W/D与CLP组相比较有明显下降(P<0.05)。CLP组、TanⅡA组的肺组织SOD和GSH活性较Sham组显著降低,但TanⅡA组与CLP组的肺组织SOD和GSH活性比较有明显增高(P<0.05)。CLP组、TanⅡA组的肺组织MDA含量和ROS活性较Sham组明显增高,但TanⅡA组的肺组织MDA含量和ROS活性较CLP组低(P<0.05)。Western blot结果示,CLP组、TanⅡA组肺组织γ-GCS蛋白表达低于Sham组(P<0.05),而TanⅡA组γ-GCS蛋白较CLP组则明显增加(P<0.05)。结论丹参酮ⅡA可促进γ-GCS蛋白表达,增加GSH的生成,抑制炎性反应,抑制细胞内脂质过氧化,清除氧自由基,从而减轻脓毒症大鼠的肺损伤。Objective: To study the lung protective effects and mechanism of Tanshinone ⅡA on acute lung injury in rats with sepsis. Methods: 24 healthy adult wistar rats were randomly divided into 3 groups including sham control group,cecal ligation and puncture operation group(CLP) and Tanshinone ⅡA group. Lung tissues were collected at the end of the experiment. Lung tissue wet/dry quality ratio(W/D),the super oxide dismutase(SOD) activity,reactive oxygen species activity(ROS),Glutathione(GSH) activity and malondialdehyde(MDA)content of lung tissue homogenates were determined. Morphological changes of lung tissues were observed under optical microscope,the relative expressions of γ-GCS protein in lung tissues were detected by western blot. Results: Sham group showed the normal structure of lung tissues and the clear structure of the alveolar under the optical microscope;CLP group showed hyperemia and edema of lung tissues,a large number of inflammatory cells infiltration,broadening alveolar interval,collapse and atelectasis of the alveolar;Tanshinone Ⅱ A group showed that the interstitial lung tissues were slightly thickened,a small amount of inflammatory cells were infiltrated,the inflammatory pathological changes of lung tissues were significantly reduced compared with CLP group. The W/D of lung tissues in CLP group and Tanshinone ⅡA group were higher than Sham group,but those of lung tissues in Tanshinone IIA group was significantly lower compared with CLP group(P〈0.05). The activity of SOD and GSH of lung tissues in CLP group and Tanshinone Ⅱ A group significantly decreased compared with Sham group,but those of lung tissues in Tanshinone ⅡA group was significantly higher compared with CLP group(P〈0.05). The MDA content and ROS activity of lung tissues in CLP group and Tanshinone Ⅱ A group significantly increased compared with Sham group,but those of lung tissue in Tanshinone Ⅱ A group was lower than CLP group(P〈0.05).
