双侧自体耳软骨镜像移植作为鼻中隔延伸移植物的临床应用

摘　　要：目的 探讨双侧自体耳软骨镜像移植作为鼻中隔延伸移植物的应用方法.方法 将切取的双侧耳软骨镜像重叠固定后,置于鼻中隔软骨尾端一侧或者骑跨于鼻中隔尾端,缝合固定,以延长并抬高鼻中隔软骨.结果 本组共206例患者,术后均获得良好的效果;随访6 ～12个月,鼻中隔获得强有力的支撑,鼻尖突出度、旋转度和形状维持良好.结论 双侧自体耳软骨镜像移植作鼻中隔延伸移植物,可获得良好的鼻中隔支撑力,增加了对鼻尖突出度、旋转度和形状的掌控,提高了鼻整形术后的恢复效果.建议临床推广应用.Objective To search a new method of bilateral conchal cartilage graft as septal extending graft in rhinoplasty. Methods After mirroring overlaying and fixation, the harvested bilateral eonchal cartilage was placed to the lateral ending of nasal septal cartilage or to straddle the ending of nasal septal cartilage and then was fixed and sutured to extend and raise the nasal septal cartilage. Results After 6 - 12 months follow- ing up, 206 patients obtained a satisfactory results, like strong support of nasal septum and the prominent nasal tip with good rotation and shape. Conclusion Bilateral conehal cartilage mirroring graft was a kind of modified septal extending graft, which can better control the standout, rotation, shape of nasal tips, to acquire better outcomes after surgery. We recommend it can be spreading in clinic.
