高强度聚焦超声消融子宫肌瘤患者腹壁损伤的影响因素研究

摘　　要：目的 探讨高强度聚焦超声（HIFU）消融治疗子宫肌瘤引起患者前腹壁声通道区损伤的影响因素.方法 50例子宫肌瘤患者,HIFU消融治疗前和治疗后1周行MRI检查.治疗后根据前腹壁是否水肿分为前腹壁水肿组和正常组.分析前腹壁水肿区体积与消融治疗参数、肌瘤相关因素的相关性.结果 HIFU治疗子宫肌瘤引起前腹壁声通道区组织水肿38例为水肿组,发生率为76％,水肿体积7.46～630.63 cm3;正常组12例.两组辐照时间、治疗总能量差异有统计学意义（P＜0.05）,两组间单位投射时间、前腹壁至肌瘤后缘距离的差异无统计学意义.前腹壁水肿体积与超声辐照时间、治疗总能量、单位投射时间、前腹壁至肌瘤后缘距离呈正相关（r值分别为0.423、0.452、0.373、0.389,P＜0.05）.结论 HIFU对子宫肌瘤辐照时间、治疗总能量是引起前腹壁损伤的影响因素.Objective To explore the influence factors of anterior abdominal wall injury caused by high intensity focused ultrasound（HIFU） ablation in treatment of uterine leiomyoma.Methods Fifty patients with uterine fibroids in total treated by HIFU were enrolled in this study,and then their MRI images were retrospectively collected.The patients were divided into the edema group and normal group.The relationship between the measured volume of the abdominal wall edema and the treatmentrelated parameters and fibroid-related information were analyzed.Results The incidence of anterior abdominal wall edema caused by HIFU treatment was 76％ （38/50）,and the edema volume range was 7.46～630.63 cm3.There were significant differences in exposure time and total treatment dose between the edema group and the normal group （P＜0.05）.There was no statistical differences in the sonication time per unit and the distance from the rear edge of fibroids to the anterior abdominal wall between the two groups.The volume of anterior abdominal wall edema was positively correlated with exposure time,total treatment dose,sonication time per unit and the distance between the rear edge of fibroids and the anterior abdominal wall （r=0.423,0.452,0.373,0.389,P＜0.05）.Conclusion The exposure time and the total treatment dose were the influence factors of anterior abdominal wall injury caused by HIFU ablation in treatment of uterine leiomyoma.
