超声造影评价经皮微波凝固治疗肝癌的疗效价值

摘　　要：目的探讨超声造影评价经皮微波凝固治疗肝癌近期疗效价值。方法 27例原发性肝癌患者,共31个病灶,对其经皮微波凝固治疗后行常规超声和超声造影检查,评价肿瘤灭活、残存或复发情况。结果 31个病灶中,26个病灶在造影各时相均未见增强,提示肿瘤完全灭活,但其中1个随访2个月增大;5个病灶动脉相治疗区残存增强病灶,门脉相及延迟相造影清退,提示有残存肿瘤,但其中1个随访12个月未见复发;超声造影诊断准确性为93.5%,敏感性为80.0%,特异性为96.1%。结论超声造影可以动态显示局部治疗区域的微循环特点及残存病灶形态,为再次补充治疗提供导向作用。Objective To investigate the value of contrast-enhanced ultrasound （CEUS） in evaluating the therapeutic effect of percutaneous microwave coagulation （PMCT） for primary hepatocellular carcinoma.Methods Twenty-seven patients （31 lesions） with hepatocellular carcinoma were included.Each patient underwent routine ultrasound and CEUS after PMCT.CEUS was performed to assess the tumor inactivation,residual and relapse.Results After PMCT,26 of 31 lesions showed no enhancement as examined by CEUS,which indicated complete tumor inactivation.The rest five lesions showed local enhancement in arterial phase and regression in portal and delayed phase,which indicated tumor residual.The diagnostic accuracy,sensitivity and specificity of CEUS were 93.5％,80.0％ and 96.1％,respectively.Conclusion CEUS can dynamically display microcirculation characteristics and various forms of local residual lesions,providing guiding role for the complementary therapy.
