彩色多普勒超声在经上肢静脉置入中心静脉导管术后随访中的应用

摘　　要：目的探讨彩色多普勒超声在经上肢静脉置入中心静脉导管(PICC)术后随访中的应用价值。方法 67例PICC患者,超声观察导管在血管内位置及走行、所在静脉管腔情况及血流状态。结果 67例患者超声检查均可显示导管。54例PICC导管及其所在静脉未见异常,表现为光滑平行管状高回声,所在静脉血流通畅,无血栓形成;10例导管所在静脉有血栓形成,表现为静脉增宽,内可见低回声物,血流信号变窄或完全无血流信号;2例导管壁周缘有附着物形成,表现为附壁较小低回声物;1例导管腔内可见堵塞物,表现为导管内较小低回声。结论超声检查可对经上肢静脉PICC术后患者随访观察。术后血栓形成是其主要并发症。Objective To investigate the clinical value of color Doppler ultrasonography in postoperative follow-up of peripherally inserted central catheter （PICC） through upper extremity vein.Methods Sixty-seven patients who received PICC underwent ultrasonography.The position and direction of intravascular catheter,the situation of venous lumen and blood-flow were observed.Results The catheter could be displayed by color Doppler ultrasonography in all 67 cases.The catheter and the located vein were normal in 54 cases.Venous thrombogenesis were found in 10 cases,which showed the widened lumen and hypoechoic spots in the lumen,the blood flow signal was narrowed or disappeared as showed by color Doppler ultrasonography.Attachments in the peripheral wall of catheter formed in 2 cases,which appeared as small wall-attached hypoechoic object.Blockage of catheter lumen was found in 1 case,which appeared as small hypoecho.Conclusion Color Doppler ultrasonography can be applied to the postoperative follow up for patients who received PICC.
