摘 要:目的分析黑白与彩色多普勒阴道超声不同检查时间对早孕胚胎绒毛超微结构的影响。方法选取人工流产的早孕者120例,随机分为两组,彩超组60例予阴道彩色多普勒超声检查,其中检查5 min者20例,10 min者18例,15 min者22例。黑白组60例予阴道黑白B超检查,其中检查5 min者19例,10 min者19例,15 min者22例。对比两组胚胎绒毛超微结构变异率及绒毛DNA单链断裂和双链断裂的情况。结果检查5 min和10 min,两组胚胎绒毛超微结构变异率差异无统计学意义(P>0.05);检查15 min,彩超组绒毛超微结构变异率高于黑白组(P<0.05)。检查5 min,两组DNA单链和双链断裂情况比较差异无统计学意义(P>0.05)。检查10 min及15 min,彩超组绒毛DNA单链、双链断裂均较黑白组增加,差异均有统计学意义(P<0.05)。彩超组:绒毛超微结构变异率,5 min组和10 min组无差异,15 min组高于10 min组。随着检查时间延长,DNA单链、双链断裂增加,5 min组<10 min组<15 min组。黑白组:检查15 min内,绒毛超微结构变异率无差异。15 min组DNA单链、双链断裂较10 min组增加(P<0.05)。结论阴道黑白超声检查15 min内,对胚胎绒毛超微结构影响较小。阴道彩色多普勒超声检查超过10 min,对胚胎绒毛超微结构影响会增加。超声检查选择应结合临床情况,并控制检查时间。Objective To analyze the effect of black and white and color Doppler transvaginal ultrasound at different examination time on ultrastructure of trophonema in early embryo. Methods 120 patients in early pregnancy who requested artificial abortion were selected, and randomly divided into two groups. Color Doppler group(CD group,n=60)was examined by color Doppler transvaginal ultrasound, with 20 cases for 5 min, 18 cases for 10 min, and 22 cases for15 min. Black and white group(BW group, n =60) was examined by black and white transvaginal ultrasound, with 19 cases for 5 min, 19 cases for 10 min, and 22 cases for 15 min. The aberration rates of ultrastructure and DNA(singlestrand break, SSB)and(double-strand break,DSB) of trophonema in two groups with different examination time were compared. Results Examined for 5 min and 10 min, the differences of aberration rates of ultrastructure between two groups were not significant(P 〉0.05).Examined for 15 min, the aberration rate in CD group was higher than that in BW group(P 〈0.05).Examined for 5 min, the differences of DNA SSB and DSB between two groups were not significant(P 〉0.05).Examined for 10 min and 15 min, DNA SSB and DSB in CD group were increased than those in BW group(P〈0.05). CD group :There were no significant differences of the aberration rates of ultrastructure between embryo examined for 5 min and 10 min; the aberration rates were increased when examined for 15 min. With the examination time prolonged, DNA SSB and DSB were increased, 5 min group 〈10 min group 〈15 min group.BW groups : Examined within 15 min, the aberration rates of ultrastructure were had no difference. When examined for 15 min, DNA SSB and DSB were increased than those examined for 10 min(P 〈0.05). Conclusion Examined with black and white Doppler in15 min, there was little effect on ultrastructure of trophonema in embryo.
