意识指数和脑电双频谱指数监测在小儿全身麻醉诱导中的应用

摘　　要：目的探讨意识指数(index of consciousness,IoC)在小儿全身麻醉诱导时镇静深度监测中的应用价值。方法 25例择期手术患儿,应用丙泊酚等进行麻醉诱导,麻醉诱导期间测试患儿意识反应,镇静深度采用镇静/警觉评分(observers assessment of alertness/sedation scale,OAA/S),记录OAA/S评分即刻的IoC,并与脑电双频谱指数(bispectral index,BIS)进行比较。结果25例中有效数据17例;OAA/S为0分时IoC值(44.82±8.47)、BIS值(39.00±14.44)均低于OAA/S为5分时(IoC值为96.65±2.64,BIS值为90.24±5.83),差异有统计学意义(P<0.01),且以上2个时间点IoC值均大于BIS值(P<0.05);OAA/S与IoC(r=0.871,P=0.000)、BIS(r=0.864,P=0.000)以及IoC与BIS(r=0.805,P=0.002)均呈正相关。结论 IoC值可反映丙泊酚麻醉诱导期间小儿镇静深度。Objective To evaluate the value of index of consciousness（IoC）to monitoring the depth of anaesthesia in anaesthesia induction in children.Methods Twenty-five children receiving operation under propofol anaesthesia induction were observed the consciousness,the depth of aesthesia was evaluated by observers assessment of alertness/sedation scale（OAA/S）,and the IoC and BIS values were recorded during OAA/S 5to 0.Results In 17 patients with valid data,IoC and BIS values were 44.82±8.47,39.00±14.44 when OAA/S was 0,significantly lower than those when OAA/A was5（96.65±2.64,90.24±5.83）（p〈0.01）,and IoC values were higher than BIS values at both time points（p〈0.05）.OAA/S was positively correlated with IoC（r=0.871,P=0.000）and BIS（r=0.864,P=0.000）,and IoC was positively correlated with BIS（r=0.805,P=0.002）.Conclusion IoC can reflect the depth of anaesthesia in propofol induction in children.
