人β防御素3在胎膜早破妊娠妇女中的表达及其意义

摘　　要：目的:探讨人β防御素3(h BD3)在胎膜早破妊娠妇女中的表达及其临床意义。方法:随机选择2013年1—12月于湖北省孝感市中心医院妇产科住院分娩的初产胎膜早破患者45例作为胎膜早破组,取同期正常妊娠妇女45例作为对照组。应用免疫组织化学SP法检测胎膜组织中h BD3的表达,酶联免疫吸附法检测血清肿瘤坏死因子α(TNF-α)和h BD3的含量。结果:胎膜早破组h BD3阳性33例;对照组h BD3阳性13例,2组阳性率比较差异有统计学意义(P<0.05);与对照组比较,胎膜早破组血清TNF-α、h BD3在入院及分娩时均升高(P<0.05),且胎膜早破组分娩时血清TNF-α、h BD3较入院时均升高(P<0.05)。结论:胎膜早破时h BD3可能是在TNF-α的介导下参与了抗微生物的天然免疫反应。Objective： To investigate the expression and its clinical significance of human beta-defensin-3（h BD3）in premature rupture of fetal membranes of pregnant women. Methods：Randomly selected 45 cases who delivered in the department of obstetrics and gynecology of our hospital and diagnosed primiparous patients with preterm premature rupture of membranes as PPROM group,and selected 45 cases of normal pregnancy during the same period as control group. Detected the expression of h BD3 in fetal membranes by immunohistochemical,and determined serum TNF-α,h BD3 content by enzyme linked immunosorbent assay. Results：Premature rupture of the epithelial cells observed in the brown staining was positive expression. PPROM group beta defensin 3（＋）expression in 33 cases,12 cases were negative; the control group beta defensin3（＋） expression in 13 cases,32 cases were negative,there was significant difference between two groups（P〈0.05）.Compared with the control group,serum TNF-α,h BD3 increased significantly at admission and delivery of premature rupture of membranes（P〈0.05）,and PPROM group serum TNF-α,h BD3 delivery were higher than the admission（P〈0.05）.Conclusions：h BD3 may be involved in the natural immune responses against microorganism mediated by TNF-α.
