经腹与经阴道超声在异位妊娠早期诊断中的效果比较

摘　　要：目的比较经腹与经阴道彩色超声对宫外孕早期诊断的效果。方法选择入院并经临床探查与病理检查确诊的宫外孕患者53例,所有患者均具有经腹部超声与经阴道彩色多普勒超声的早期检查结果,并对比分析其临床诊断效果。结果 53例患者中阴道超声检查发现附件区包块49例,假孕囊23例,卵黄囊24例,胎心16例,胚芽15例,总检出率为94.34%,腹部超声检查发现附件区包块39例,假孕囊9例,卵黄囊11例,胎心5例,胚芽5例,总检出率为73.58%,经阴道彩色多普勒超声早期诊断宫外孕的诊断符合率明显高于经腹部超声诊断,且对附件区包块、宫内假孕囊、卵黄囊、胎心及胚芽的检出率均明显高于经腹部超声诊断,差异具有统计学意义(P<0.05)。患者通过腹部超声与阴道超声检查检出时间比较,阴道超声为(39±8.7)d明显优于腹部超声(43±6.2)d,差异具有统计学意义(P<0.05)。结论经阴道彩色多普勒超声对宫外孕早期诊断具有更为理想的检出效果,对各分项检出率也均优于腹部超声,可在临床推广应用。Objective To compare the effect of transabdominal and transvaginal color Doppler ultrasound in early diagnosis of ectopic pregnancy. Methods 53 cases admitted to hospital were confirmed the diagnosis of ectopic pregnancy by clinical exploration and pathology examination. All patients had the early findings of abdominal ultrasound and vaginal color Doppler ultrasound. The clinical diagnosis effect was observed. Results Vaginal ultrasound examination of 53 cases found 49 cases of adnexal masses, 23 cases of false pregnancy sac, 24 cases of yolk sac, 16 cases of fetal heart and 15 cases of germ. The total positive rate was 94.34%. Abdominal ultrasound examination found 39 cases of adnexal masses, 9 cases of false pregnancy sac, 11 cases of yolk sac, 5 cases of fetal heart and 5 cases of germ. The total positive rate was 73.58%. The diagnose accordance rate of transvaginal color Doppler ultrasound in early diagnosis of eetopie pregnancy was significantly higher than that of abdominal ultrasound. The adnexal masses, Miyamauehi Kari, yolk sac, fetal heart and germ detection rates were significantly higher than that of abdominal ultrasound, the difference has statistical significance （P〈0.05）. The detection time of transvaginal ultrasound （39±8.7） d was superior to abdominal ultrasound （43±6.2） d （P〈0.05）. Conclusion Transvaginal color Doppler ultrasound for early diagnosis of ectopie pregnancy has more ideal effect of detection, with better detection rate of each item than abdominal ultrasound.
