手术室整体护理对肿瘤患者心理情绪及预后的影响

摘　　要：目的探讨手术室对手术治疗的恶性肿瘤患者实施整体护理的临床效果。方法纳入行手术治疗的恶性肿瘤患者288例,以2013年6月前的144例患者作为对照组,之后的144例患者作为干预组,对照组实施常规外科护理;干预组实施整体护理,护理前后对两组肿瘤患者进行汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)、汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)、Karnofsky功能状态和肿瘤病人的生活质量(QOL)评分,比较两组患者在治疗过程中出现生命体征异常的频率和发生并发症的情况。结果手术前两组患者HAMD、HAMA、KPS及QOL分数无明显统计学意义(P>0.05);手术后,干预组的HAMD及HAMA分数都显著低于对照组(P<0.05),KPS及QOL分数都显著高于对照组(P<0.05)。干预组患者手术后的生命体征异常发生率及并发症发生率都显著低于对照组(P<0.05)。结论手术室实施整体护理,能有效管理肿瘤患者心理状态和情绪,明显改善患者的预后,提高患者的生存质量,可减少患者手术后不良事件的发生。Objective To study the clinic value of holistic nursing in operation room.Methods 288 consecutive patients were divided into the before the June 2013 of 144patients as control group and after the June 2013 144 patients as intervention group.The routine care was given in the control group,while holistic nursing care was given in the intervention group.The HAMD,HAMA,Karnofsky andQOL score before and after care and the occurrence of adverse events were observed.Results After operation,HAMD score and HAMA score of the patients in intervention group were significantly lower than those in the control group（P〈0.05）.KPS score and QOL score of the patients in intervention group were significantly higher than those in the control group（P〈0.05）.The incidence of adverse events in intervention group after operation were significantly lower than those in the control group（P〈0.05）.Conclusion In operation room,holistic nursing care can effectively manage the psychological state of patients with tumor and emotional,and significantly improve the prognosis and quality of life of patients,and reduce the incidence of adverse events.
