摘 要:目的 探讨超声定量组织多普勒速度成像技术(QTVI)评价正常胎儿心房壁运动的临床价值.方法 运用QTVI对52胎胎龄21 ~ 28周正常胎儿心脏进行检测,在心尖四腔心切面,对左右心房壁4个观察点的速度曲线进行分析,测量收缩波峰值速度(Vs),舒张早期运动速度(Ve),舒张晚期运动速度(Va),比较左右心房壁心肌运动速度差异.结果 正常胎儿心房的组织多普勒速度曲线主要由3个峰组成,即收缩期s峰、舒张早期e峰、舒张晚期a峰.左右心房壁基底段与中间段速度曲线均显示e峰小于a峰.正常胎儿心房壁运动速度的变化趋势为基底段>中间段;左右心房比较,右心房游离壁基底段和中间段Vs、Ve、Va的运动速度大于左心房相应节段,差异均有统计学意义[基底段Vs:(3.40 ±0.89) cm/s比(2.19 ±0.64)cm/s、基底段Ve:(2.26±1.16) cm/s比(1.14±0.62) cm/s、基底段Va:(6.34±1.09) cm/s比(3.70±1.19) cm/s;中间段Vs:(1.95±0.87) cm/s比(1.43±0.62) cm/s、中间段Ve:(0.99 ±0.64) cm/s比(0.43±:0.29) cm/s、中间段Va:(3.37±1.31) cm/s比(1.39±0.78) cm/s,均P<0.05].结论 QTVI用于评价胎儿不同房壁心肌运动及局部功能是可行的.Objective To assess the value of quantitative tissue velocity imaging(QTVI) in assessing the auricular wall motion of normal fetuses.Methods Fifty two normal fetuses from the 21st gestation week to the 28th gestation week underwent QTVI.The peak velocities were measured in the basal and mid parts of the left and right free auricular wall on the apical four-chamber view.The peak sustained systolic velocity (Vs),peak early diastolic (Ve) and peak late diastolic velocity (Va) were measured and compared between left and right auricular.Results The tissue velocity imaging curve consisted of "s","e" and "a" peak.The peak velocities of "a" peak was higher than that of "e" peak in both the basal part and the mid part in the left and right auricular.The peak velocities of the basal part were higher than those of the mid part in the left and right free auricular wall.The peak velocities of right free auricular wall were higher than those of left free auricular wall in both the basal part [Vs:(3.40 ±0.89) cm/s vs (2.19 ±0.64) cm/s,Ve:(2.26 ± 1.16) cm/s vs (1.14 ±0.62) cm/s,Va:(6.34 ± 1.09) cm/s vs (3.70±1.19) cm/s] and the mid part [Vs:(1.95 ±0.87)cm/s vs (1.43 ±0.62)cm/s,Ve:(0.99±0.64)cm/s vs (0.43 ±0.29) cm/s,Va:(3.37 ±1.31) cm/s vs (1.39 ±0.78)cm/s] (all P〈 0.05).Conclusion QTVI is feasible in assessing the auricular wall motion and function of the normal fetuses.
